Basınç Ülserleri Tedavi Maliyetleri Etkililik Analizi; Konvansiyonel ve Modern Yara Bakım Tedavi Maliyetlerinin Karşılaştırılması
Amaç: Basınç ülseri, yataklı tedavi kurumları için hem bir kalite göstergesi hem de ciddi bir maliyet yükü oluşturmaktadır. Tedavinin ve hastanede kalış süresinin uzaması, özel bakım gerektireceğinden maliyeti artırır. Çalışmanın amacı, basınç ülserinin tedavi maliyetlerine olan etkisini analiz etmek; tedavi yöntemlerinin evrelerine göre maliyetlerini karşılaştırarak önleyici faaliyetlerin önemini vurgulamaktır. Gereç ve Yöntemler: Tanımlayıcı tipte olan bu araştırmanın kapsamını, Mayıs 2012-Mayıs 2013 tarihleri arasında Akdeniz Üniversitesi Hastanesi klinikleri ve yoğun bakım servislerinde takip edilen basınç ülserli hastalar n=569 oluşturmuştur. Hasta dosyalarından elde edilen veriler sınıflandırılarak SPSS 16 Windows programına aktarıldı. Hastaların demografik özellikleri tanımlayıcı istatistikler kullanılarak analiz edildi. Ayrıca karşılaştırmalı analizlerde, t-testi, varyans ve post hoc yöntemleri uygulandı. Bulgular: Basınç ülserlerinin olgu başına maliyeti 4.615 dolar olarak hesaplanırken, 569 olgunun toplam maliyeti ise 2.630.917 dolar olarak hesaplanmıştır. Modern yara bakım ürünleri kullanılan hastalarda hastanede yatış süresi, konvansiyonel tedavi alanlara göre 2 gün daha kısadır. Modern yara bakım ürünleriyle tedavinin maliyeti ortalama 67 dolar iken konvansiyonel tedavi maliyetinin 19 dolar olduğu tespit edilmiştir. Sonuç: Çalışmada hesaplanan basınç ülseri görülme sıklığına göre Türkiye’de yılda 308.796 hastada basınç ülseri gelişebileceği ve bunun maliyetinin yaklaşık 1 milyar 425 milyon dolar olduğu tahmin edilmektedir. Önleyici faaliyetlerin geliştirilmesi ve olguların erken tespiti basınç ülserlerinden kaynaklanan mali kayıpları oldukça azaltacaktır
The Cost Analysis of Pressure Ulcer Treatment; The Treatment Cost Comparison of Conventional and Modern Wound Care
Objective: Pressure ulcer is an important quality indicator and an overlooked cost burden for the hospital. Prolongation of hospital stay duration and also the required special care are the main causes of increasing cost. Pressure ulcers continue to be a major problem despite the possibility of prevention with simple measures. The aim of this study was to analyse the impact of pressure ulcer treatment costs and to emphasize the importance of the preventive actions by calculating the costs. Material and Methods: The identifying universe of this research were patients n=569 with a pressure ulcer, who were followed up in the clinics and intensive care units of Akdeniz University Hospital between May 2012 and May 2013. The information obtained from the relevant patient documents and hospital archives were recorded in SPSS 16 Windows. The demographic characteristics of the patients were analysed by descriptive statistics. In addition, the t-test, variance and post hoc methods were used for comparative analysis.Results: The total cost per case was $ 4,615 in pressure ulcer and the total cost of the 569 cases were estimated to be $ 2,630,917. The cost of treatment with modern wound care products at $ 67 was found to be higher than the conventional treatment cost of $ 19. The numbers of days of hospitalization for the patients treated with modern wound care products were 2 days shorter than for the patients that received conventional treatment. Treatment cost in the early stage was lower than for the advanced stage. Conclusion: According to the incidence rates obtained from this study, it is estimated that annually 308,796 patients in Turkey are likely to develop pressure sore with a cost of 1 billion 425 million US Dollars. Development of preventive activities, raising awareness among the healthcare staff, and early detection of the cases will substantially reduce the financial loss arising from pressure ulcer.
