Yetişkin hastada intestinal malrotasyon

İntestinal malrotasyonlar embriyojenik dönemde orta barsağın fizyolojik herniasyonu sırasında oluşurlar. Malrotasyonların farklı formlarından dolayı tanısı oldukça zordur. Malrotasyonlar sıklıkla konjenital peritoneal bantlarla ilişkilidirler. Bu bantların en yaygını Ladd’s bantlarıdır. Bunlar çekumdan subhepatic alana kadar uzanabilirler ve duodenuma baskı yapabilirler. Bunlar infantlarda daha sıktırlar, ancak yaşamın herhangi bir döneminde görülebilirler. İnfantlarda genellikle safralı kusma şeklinde klinik görünüm mevcutken, yetişkinlerde bu durum oldukça değişkendir. Tedavide altın standart Ladd’s prosedürüdür. Biz 4 yıldır nonspefisik gastrointestinal semptomları olan, tomografide malrotasyondan şüphelenilen ve Ladd’s prosedürü ile tedavi edilen 23 yaşındaki bayan hastayı sunduk.

Intestinal malrotation in an adult patient)

Intestinal malrotations occur during the physiological herniation of midgut in the embryonic period. Diagnosis is difficult because of the different forms of malrotations. They are often associated with congenital peritoneal bands. The most common of these bands is Ladd’s band. They can lie down from the caecum to the sub-hepatic area, and cause compression of the duodenum. These are more frequent in infants yet can occur at any age. Clinical appearance in the adults are quite variable while it usually occurs in infancy as bilious emesis. In adults, the most common symptom is recurrent bouts of colicky abdominal pain. The gold standart treatment is Ladd’s procedure. We report a case of 23-year-old female patient with a 4-year history of non-specific gastrointestinal symptoms and suspected malrotation on the tomography. She was treated by Ladd’s procedure.
