Yenidoğanlarda ağır metabolik asidozun nadir bir nedeni: aort koarktasyonu

Metabolik asidoz ve şok yenidoğan döneminde sık karşılaşılan ve hayatı tehdit eden sorunlardandır. Doğum asfiksisi, sepsis, patent duktus arteriozus gibi birçok neden bu kliniğe neden olabilirken; yenidoğanlarda pek akla gelmeyen ve nadir bir neden olan aort koarktasyonunda, ilk günlerdeki müphem bulgular atlanıldığında mortalite ve morbidite çok artmaktadır. Kalp yetmezliği ve şok gelişmeden önce tanının konulması ve bu bulgularla gelen hastalarda aort koarktasyonu olabileceğinin akılda tutulması gerekmektedir. Biz burada ağır metabolik asidoz ve şokla gelen ve aort koarktasyonu tanısıkonan bir hastayı sunduk.

Coarctation of the aorta, a rare cause of severe metabolic acidosis and shock in the newborn

Metabolic acidosis and shock in the neonatal period are common and life threatening findings and often caused by conditions such as asphyxia, sepsis and patent ductus arteriosus. Coarctation of the aorta is a rare cause of metabolic acidosis and its mortality and morbidity increases rapidly with a delay in the diagnosis. The possibility of this rare diagnosis should be kept in mind by the neonatologist before the initiation of symptoms. Here we present a newborn infant with severe metabolic acidosis and shock who was then diagnosed with coarctation of the aorta following a successful resuscitation.
