Varis dışı üst Gastrointestinal sistem kanamalarının değerlendirilmesi
Amaç: Üst gastrointestinal kanama, ciddi morbidite ve mortalitesi olan yaygın bir medikal problemdir. Bu çalışmada varis dışı üst Gastrointestinal kanamaların etiyoloji ve epidemiyolojisinin araştırılması amaçlandı. Yöntem: Hastanemizin gastroenteroloji birimi Ekim 2008-Ekim 2010 kayıtları geriye dönük tarandı. 108 (74 erkek, 34 kadın) akut varis dışı üst gastrointestinal kanama hastası belirlendi. Bu hastaların yaş ve cinsiyet dağılımı, anamnez verileri, endoskopi bulguları, geliş tarihleri kaydedilerek istatistikî analizleri yapıldı. Bulgular: Hastaların yaş ortalaması 57,4 idi. Endoskopik görünümlerine göre başlıca kanama nedenleri duodenal ülser %34,3, gastrik ülser %27,8, gastrik ve duodenal ülser birlikteliği %2,8, özofajit %5,6, eroziv gastrit %2,8 anjiyodisplazi %1,9, Mallory-Weiss %1,9 idi. Duodenal ülser ve gastrik ülser hastalarının yaş ortalamaları sıra-sıyla 50,81±20,4, 63,87±16,7 idi (p=0,02). %48inde non-steroid anti-inflamatuvar ilaç (asetil salisilik asit dahil) kullanım öyküsü vardı. Endoskopik tedavi olarak hastaların % 10,2sine skleroterapi uygulanmıştı. %34,3 hastada bir veya daha fazla ek hastalık vardı. Hastaların çoğu ilkbahar (%30,6) ve yaz (%40,7) aylarında gel-mişti. Sonuç: Peptik ülser, en sık varis dışı üst gastrointestinal kanama nedenidir. Duodenal ülser, gastrik ülserden daha sık ve daha genç yaşlarda görülmektedir. Hastala-rının yaklaşık yarısında NSAİ ilaç kullanım hikâyesi var-dır. Uzun süreli NSAİ kullanacak ya da yüksek riske sahip hastalara NSAİ ilacın yanında gastroprotektif tedavi de verilmelidir.
The evalution of non-variceal upper gastrointestinal bleeding
Objective: Acute upper gastrointestinal (GI) bleeding is a common medical emergency and a major cause of morbidity and mortality. In the present study, we aimed to investigate etiological and epidemiological character-ristics of non-variceal upper gastrointestinal bleeding. Material and Method: We evaluated the records of 108 patients (74 male, 34 female) who presented to our hospital with acute non-variceal upper gastrointestinal bleeding between October 2008 and 2010. Characteristics of patients were evaluated in terms of age, gender, admission date, medical history, endoscopy findings. Results: Mean age was 57.4. The frequent causes of bleeding were duodenal ulcer (34.3%), gastric ulcer (27.8%), gastric and duodenal ulcers (2.8%), esophagitis (5.6%), erosive gastritis (2.8%), and others (3.8%) in endoscopic appearances. Mean ages of patients with duodenal ulcer and gastric ulcer were 50.81±20.4 and 63.87±16.7 years (p=0.02). The percentage of patients using non-steroidal anti-inflammatory drug (NSAİD) was 48%. Endoscopic sclerotherapy was made in 10.2% of patients. Comorbid diseases were present in 34.3% of the patients. The percentage of patients in the months of spring (30.6%) and summer (40.7%) was markedly higher than those in other months. Conclusions: Peptic ulcer disease remains the most common cause of non-variceal upper GI bleeding. Duodenal ulcers are more common than gastric ulcers and tend to occur at a younger age in younger people. The use of NSAID is present in about half of patients with non-variceal upper GI bleeding. Gastro-protective co-therapy should be considered in patients with high-risk or long-term use.
