Ulnar sinir tuza nöropatisinin kubital oluk düzeyinde insitu dekompresyon yöntemi cerrahi tedavisi tedavisi: 59 olgunun değerlendirilmesi

Amaç: Ulnar sinir tuzak nöropatileri (TNP), üst ekstremitede karpal tünel sendromundan sonra ikinci sıklıkta görülen tuzak nöropatilerdir. Anatomik özellikleri nedeniyle ulnar sinir en sık dirsek bölgesinde basıya uğramaktadır. Tedavisi konservatif yaklaşım veya cerrahidir. Bu çalışmadaki amacımız ulnar TNPde uyguladığımız insitu dekompresyon tekniğinin cerrahi sonuçlarını incelemektir. Materyal ve Metot: 2005-2010 yılları arasında GATA Haydarpaşa Eğitim Hastanesi Beyin Cerrahi Kliniğinde ulnar TNP tanısı alan 59 hasta opere edildi. Hastaların 37si erkek, 22si ise bayan idi. Yaş ortalaması 34,4 (22- 62 yaş) idi. Hastaların tümüne lokal anestezi altında kubital oluk düzeyinde insitu dekompresyon yapıldı. Bulgular: Hastaların %91,5inin şikayetleri ameliyattan sonra belirgin derecede düzeldi. 5 hasta cerrahiden fayda görmediğini ifade etti. Bu hastaların 3ü Tip-II diyabet, 1i romatoid artrit tanısı ile takip ediliyordu. Bir hastada ise ulnar sinirin aşırı dekompresyonuna bağlı instabilite geliştiği belirlendi. Ulnar instabilite gelişen hastamız dışında hiçbir hastada cerrahi komplikasyon gelişmedi ve hastaların şikayetleri ameliyattan sonra artmadı. Sonuçlar: İnsitu dekompresyon kubital oluk düzeyindeki ulnar sinir TNPsinde yaygın olarak kullanılan güvenilir bir cerrahi yöntemdir. Sistemik tutulumlu hastalığı olanlar ve diabetli olgularda uygulanan cerrahi yüz güldürücü olmadığından bu olgular preoperatif dönemde iyi değerlendirilmelidir.
