Normal ventrikül fonksiyonları ile giden pulmoner arterden kaynaklanan sol koroner arter anomalili (ALCAPA) bir olgunun tanı koydurucu ekokardiyografik bulguları
ALCAPA nadir görülen bir doğumsal kalp defektidir. Asemptomatik olup rutin inceleme esnasında üfürüm saptanması nedeniyle kliniğimize başvuran hastaya yapılan ekokardiyografik (EKO) incelemede sol koroner arter görüntülenemezken, sağ koroner arter çapının genişlediği, mitral kapakta birinci dereceden mitral yetersizlik ve interventriküler septum (İVS) üzerinde çok sayıda sol sağ şant ile uyumlu olabilecek akım saptandı. Yapılan anjiyokardiyografik inceleme ile sol koroner arterin pulmoner arterden köken aldığı, İVS üzerinde defekt olmadığı saptanan hastaya ALCAPA tanısı kondu. ALCAPA'lı hastalarda ekokardiyografide İVS üzerinde renkli Doppler USG inceleme ile akım saptanması uyarıcı olmalıdır. Pulmoner hipertansiyonun bulunmadığı, sol kalp boşluklarında genişleme olmadan mitral yetersizliğin görülmesi, İVS üzerinde saptanan bu akımın VSD'ye ait olmadığı yönünde önemli ipucudur. Sağ koroner arter çapının genişlemiş olması ve romatizmal kalp hastalığına bağlı olmayan mitral yetersizliğinin olması da diğer tanı kriterleri olarak akılda tutulması gerekir.
Diagnostic echocardiographic findings of a case with anomalous left coronary artery from pulmonary artery (ALCAPA) presenting normal ventricular functions
ALCAPA is a rare congenital heart defect. We performed echocardiography to an asymptomatic patient who was referred to pediatric cardiology unit after evaluation of a murmur in health care visits. The echocardiography showed a dilated right coronary artery (RCA), a grade 1 mitral regurgitation and many flow on interventricular septum (IVS) concordant with shunts from left to right. Left coronary artery (LCA)was not demonstrated by echocardiography. Patient was diagnosed as ALCAPA upon evaluation of angiocardiographic investigation that revealed no defect on IVS and also LCA was originated from pulmonary artery. A flow demonstrated by conventional color Doppler USG on IVS in patients with ALCAPA, should be a caution for practicioners. Mitral regurtation without left heart cavity dilatation and pulmonary hypertension is an evidence for opinion about flow should not be a cause of VSD demonstrated on IVS. Furthermore, RCA dilatation and a mitral regurtation not related to rheumatic heart disease shoul be kept in mind as other diagnostic criteria.
