Akut pankreatitde prognozu ve hastanede yatış süresini etkileyen etyolojik faktörlerin değerlendirilmesi

Akut pankreatit, pankreasın inflamatuar hastalığı olarak tanımlanmaktadır. Hastalığın patogenezinde çeşitli teori-ler öne sürülmekle birlikte kesin bir görüş birliği sağla-namamıştır. Prognozu değerlendirmek üzere Ranson kriterleri, Imrie skorlama sistemi, APACHE-II skalası, Balthazar skoru ve “bilgisayarlı tomografi şiddet indeksi” gibi değerlendirme ve skorlama sistemleri bulunmak-tadır. Türkiye Yüksek İhtisas Hastanesi’ne (TYİH) Haziran 2007 ile Temmuz 2009 tarihleri arasında başvurarak akut pankreatit tanısı alan hastalar retrospektif olarak ince-lendi. Hastalar etiyoloji, laboratuar parametreleri, prognoz, komplikasyonlar ve yatış süreleri açısından değerlendirildi. Akut Pankreatit tanısı ile hospitalize edilen 103 hasta çalışmaya dahil edildi. Hastaların 39 (%37,8) tanesi erkek, 64 (%62,2) tanesi kadın idi. Etiyolojik nedenler arasında en sık rastlanan neden safra taşı idi. Hastanede yatış süreleri açısından gruplar arasında anlamlı bir fark mevcut değildi. Dokuz hastada pankreatik psödokist, 2 hastada nekroz ve 1 hastada abse oluşumu saptandı. Serum AST düzeyleri ile yatış süresi arasında pozitif korelasyon olduğu saptandı. Çalışmamızda hastaneye başvuru esnasındaki serum AST değerleri ile hastanedeki yatış süreleri arasında anlamlı bir ilişki saptandı. Akut pankreatit nedeniyle hospitalize edilen hastalarda uzun dönem prognozu ve hastanede yatış süresinin önceden tahmin edebilecek yeni skorlama sistemlerine ihtiyaç vardır.

Evaluation of etiologic factors that determines prognosis and hospitalization time in acute pancreatitis

Acute pancreatitis is an inflamatuar disorder of pancreas. Although several theories put forward to explain the pathogenic events, it is still under debate. To evaluate prognosis, there are a number of evaluation and scoring systems including Ranson criteria, Imrie scoring system, APACHE-II scale, Balthazar score and BT severity index. Patients diagnosed with acute pancreatitis that admitted to Turkiye Yuksek Ihtisas Training and Research Hospital during June 2007 and July 2009 was evaluated retrospectively. Their clinical profile, laboratory parameters, prognosis, complications and hospitalization time were noted and analyzed. One-hundred and three patients with a diagnosis of acute pancreatitis were included in this study. Male/female rate was 39 (37.8%) and 64 (62.2%) respectively. Biliary pathologies were the most common identified etiologic factors in acute pancreatitis etiology. Hospitalization time was not statistically significant in groups at different etiology. Radiologic evalaution showed pancreatic pseudocyst in 9 patients, necrosis in 2 patients and abscess formation in 1 patient. Correlation between serum AST levels and hospitalisation time was found to be statistically significant. In this study, it has been shown that serum AST levels were associated with prolonged of hospitalization. Further prospective controlled studies are needed to evaluate current datas and put forward new scoring systems to predict prognosis and time of hospitalization
