KİŞİLİK BOZUKLUKLARINDA ŞEMA TERAPİ YAKLAŞIMI

Şema Terapi ŞT kişilik bozukluklarının tedavisinde etkililiği görgül çalışmalarla kanıtlanmış, yaşam boyu devam eden kronik sorunlara yönelik bütüncül bir tedavi yaklaşımıdır. Son yıllarda giderek popüler hale gelen ŞT’nin kuramsal varsayımları ve klinik etkililiği klinisyen ve araştırmacılar tarafından daha çok ilgi görmeye başlamıştır. Ancak şema terapide karşılaşılan karmaşık vakaların tedavisi birçok klinisyen için terapide tıkanmaya yol açmış konulardan biri olmuştur. ŞT'nin, daha karmaşık vakaları tedavi etmede tek başına yeterli olmadığı göz önünde bulundurulduğunda, Şema Mod Terapisi ŞMT terapistlerin tıkanmış hissettiklerinde kullanabilecekleri ŞT'nin gelişmiş bir parçası olarak görülmüştür. ŞMT geleneksel yaklaşımların başarısız olduğu özellikle karmaşık kişilik patolojisinin tedavisinde modlarla daha ileri düzeyde çalışmaya işaret eden etkili bir terapi şeklidir. Ancak ülkemizde kişilik bozuklukları için ŞMT ve ŞT’nin hem kuramsal varsayımlarını hem de bu varsayımları test eden görgül çalışmaları bütünleştiren derleme makaleleri sınırlıdır. Bu derleme makalesi alanyazındaki bu boşluğu doldurmak amacıyla, ŞMT ve ŞT Modeli’nin kuramsal ve tedavi kısmını kişilik bozuklukları çerçevesinde terapötik ittifak ve etkililik çalışmalarıyla birlikte ele almıştır. ŞMT ve ŞT’nin başta Sınır Kişilik Bozukluğu SKB olmak üzere kişilik bozuklukları için umut vaat eden bütünleştirici bir tedavi yöntemi olduğu düşünülmektedir

SCHEMA THERAPY APPROACH FOR PERSONALITY DISORDERS

Schema therapy ST is an integrative treatment method to chronic lifelong problems with effectiveness, supported with empirical studies for treating personality disorders. Theoretical assumptions and clinical effectiveness of ST, becoming increasingly popular recently, has gained more attention by clinicians and researchers. However, complex cases in ST have been difficult to treat for many clinicians. Considering that ST is not adequate by itself for the treatment of more complex cases, Schema Mode Therapy SMT has been seen as an advanced piece of ST for the therapists when they feel frustrated. SMT, referring to more advanced work with modes for the treatment of particularly complex psychopathology, is an effective therapy method. However, review articles of SMT and ST for personality disorders that integrated both theoretical assumptions and empirical studies are limited in Turkey. In an attempt to fill this gap in the literature, this review article discussed theoretical and treatment parts of SMT and ST with therapeutic alliance and effectiveness studies within the frame of personality disorders. SMT and ST appear to be a promising integrative treatment approach to personality disorders, particularly borderline personality disorder
