Yetersiz Biyopsi Sonrası Yanlış Yorumlanan Ameloblastik Fibro-Odontoma Vakası: Olgu Sunumu

Amaç: Ameloblastik fibro-odontoma (AFO) epitelyal ve mezenşimal hücrelerin proliferasyonu sonucu oluşan, nadir görülen, iyi huylu odontojenik bir tümördür. Genellikle çocuklarda görülür ve asemptomatik seyirlidir. AFO sınıflandırma bakımından aynı grupta bulunan diğer lezyonlara benzer özellikler taşımaktadır. Sunduğumuz olguda da eksik biyopsi sonucu yanlış teşhis konulan AFO vakası sunulmakta ve lezyonun histopatolojik olarak ayırıcı teşhisinde biyopsi örneklerinin eksiksiz alınmasının önemi vurgulanmaktadır. Olgu: Sağ mandibular posterior bölgesinde geçmeyen şişlik şikayetiyle kliniğimize başvuran 9 yaşındaki erkek hastada öncelikle biyopsi yapılmasına karar verildi ve ardından kitlenin teşhisi Ameloblastik Fibroma (AF) olarak konuldu. Ancak lezyonun cerrahi eksizyonu sonucu yapılan ikinci histopatolojik incelemede kitlenin AFO olduğu rapore edildi. Hastanın postoperatif dönemde yapılan kontrollerinde ise iyileşmenin sorunsuz olduğu görüldü ve herhangi bir nüksle karşılaşılmadı. Sonuç: AFO’nın literatürde odontomanın farklı bir safhası mı yoksa farklı bir patoloji mi olduğu konusunda da tartışmalar bulunmaktadır. Kesin teşhis için histopatolojik incelemenin dikkatle yapılması gerekmektedir. Ancak biyopsi kesitlerinin eksiksiz olması vakamızda karşılaştığımız gibi histopatolojik açıdan yanlış yorumlamaların önene geçecektir.

Ameloblastic Fibro-Odontoma Misinterpreted Histopathologically After Insufficient Biopsy: A Case Report

Objective: Ameloblastic fibro-odontoma (AFO) is a rare benign odontogenic tumor caused by proliferation of epithelial and mesenchymal cells. It is generally seen in children and has an asymptomatic course. In terms of classification, AFO has similar features to other lesions in the same group. In our case, the case of AFO which was misdiagnosed as a result of incomplete biopsy is presented and in the histopathological differential diagnosis of the lesion, the importance of taking biopsy samples completely is emphasized. Case: A 9-year-old male patient who applied to our clinic with a complaint of swelling that did not pass in the right mandibular posterior region was decided to take a biopsy first, and then the diagnosis of the mass was made as Ameloblastic Fibroma (AF). However, in the second histopathological examination performed as a result of surgical excision of the lesion, the mass was reported to be AFO. In the patient's postoperative controls, healing was observed to be smooth and no recurrence was encountered. Conclusion: There are also discussions in the literature about whether AFO is a different stage of odontoma or a different pathology. Histopathological examination should be done carefully for a definitive diagnosis. However, as in our case, biopsy sections should be performed completely to prevent histopathological misinterpretations.
