Bipolar Bozukluk ve Çocukluk Çağı Travması

Bipolar bozukluk, belli bir düzen olmaksızın tekrarlayan depresyon, mani ya da her ikisini de kapsayan karma ataklar ile giden ve bu ataklar arasında kişinin tamamen iyileşebildiği kronik bir bozukluktur. Travmatik yaşantıların bipolar bozuklukla ilişkisi üzerine yapılan çalışmalar, belirtiler üzerinde anlamlı, uzun süreli etkileri olduğunu göstermektedir. Psikososyal streslerin varolan bipolar bozukluğun nörobiyolojisini zaman içinde değiştirebileceği bildirilmiştir. Travmatik olayların bipolar bozukluğun yalnızca ortaya çıkışını değil, seyrini, atakların ortaya çıkışını etkilediği, bu hastalarda intihar girişimi oranını arttırdığını, madde kötüye kullanımının sıklıkla eşlik edebildiğini gösteren çalışmalar bulunmaktadır. Çocukluk çağında travmatik yaşantısı olanlarda bipolar bozukluk daha erken yaşta başlamakta, bu hastalar daha fazla sayıda duygudurum atağı geçirmekte, daha fazla ek tanı görülmektedir. Bu yazıda, çocukluk çağı travması ve bipolar bozukluk ilişkisi gözden geçirilmiştir. 

Bipolar Disorder and Childhood Trauma

Bipolar disorder is a chronic disorder in which irregular course of depressive, mania or mixed episodes or a complete recovery between episodes can be observed. The studies about the effects of traumatic events on bipolar disorder showed that they had significant and long-term effects on the symptoms of the disorder. Psychosocial stress might change the neurobiology of bipolar disorder over time. The studies revealed that the traumatic events could influence not only the onset of the disorder but also the course of the disorder and in these patients the rate of suicide attempt and comorbid substance abuse might increase. Bipolar patients who had childhood trauma had an earlier onset, higher number of episodes and comorbid disorders. In this review, the relationship between childhood trauma and bipolar disorder is reviewed.
