Endoskopik Retrograd Kolanjio Pankreatografi İşleminde Monitorize Anestezi Bakımında Deksmedetomidin-Propofol ve Remifentanil-Propofol Protokollerinin Karşılaştırılması

GİRİŞ ve AMAÇ: Endoskopi işlemleri son yıllarda sedasyon altında yapılmaktadır. Biz bu çalışmada endoskopik retrograt kolanjio pankreatografi uygulanacak hastalardaki monitorize anestezi bakımında deksmedetomidin-propofol ve remifentanilpropofol protokollerini karşılaştırmayı amaçladık. GEREÇ ve YÖNTEM: Grup deksmetedomidin-propofol'e 1 ?g/kg deksmedetomidin 10 dk'da yükleme dozu uygulandıktan sonra 0.5 ?g/kg/st deksmedetomidin infüzyonuna devam edildi. Grup remifentanil-propofol'e 0,2 ?g/kg remifentanil 5 dk'da yükleme dozu uygulandıktan sonra 0,1 ?g/kg/dk remifentanil infüzyonuna devam edildi. Deksmedetomidine-Propofol Versus Remifemtanyl-Propofol Usage in Endoscopic Retrograde Cholangio Pancreatography Procedures at Monitored Anesthesia Care

INTRODUCTION: In recent years endoscopic procedures are being under sedation. In this study we aimed to compare deksmedetomidine -propofol vs. remifentanyl-propofol sedation at monitored anesthesia care on patients who underwent endoscopic retrograde cholangio pancreatography procedure. METHODS: In the deksmedetomidine -propofol group deksmedetomidine loading dose of 1 mg/kg was administered in ten minutes. Following the loading dose, deksmedetomidine infusion rate of 0,5 mg/kg/hr was continued. In the remifentanyl-propofol group, remifentanyl loading dose of 0,2 ?g/kg was administered in five minutes and infusion of 0,1 mcg/kg/dk was continued. In both groups patients were given 3 mg/kg/hr of propofol infusion RESULTS: We found a significantly lower heart rate, systolic arterial pressure, diastolic arterial pressure in group deksmedetomidine -propofol. But controversly, apne and recovery times were found significantly higher in group deksmedetomidine -propofol than group remifentanylpropofol. DISCUSSION and CONCLUSION: At monitored anesthesia care, both deksmedetomidine -propofol and remifentanylpropofol sedation protocols are applicable. But in any case all equipments must always be ready to be able to give general anesthesia for the patient in order to control safety.
