ALIŞILMADIK BİR LAVMAN UYGULAMASINA BAĞLI REKTOSİGMOİD İSKEMİ VE PERFORASYON
Öğrenimi ve uygulaması kolay bir yöntem olması nedeniyle kendi kendine lavman uygulaması birçok hasta tarafından kabul görmüştür. Daha önceden bilinen bir hastalığı olmayan 85 yaşında bayan hasta, 3 gündür olan, önceleri alt kadranlarda daha belirgin iken son birkaç saat içerisinde tüm karına yayılan ve şiddeti giderek artan ağrı şikâyeti ile acil servise başvurdu. Hastanın öyküsünden son 4 yıldır kabızlık nedeni ile bahçe hortumu ve çeşme suyu ile ara ara kendisine lavman uyguladığı, son zamanlarda yeterince rahatlayamadığı için işlemin sıklığını artırdığı öğrenildi. Tahlil ve tetkikler sonucunda içi boş organ perforasyonu tanısı konulan ve acil şartlarda ameliyata alınan hastada işlem esnasında rektosigmoid kolonda iskemi ve perforasyon saptandı. Ameliyat sonrası gelişen çoklu organ yetmezliği nedeni ile hasta yaşamını yitirdi. Uygulamaya bağlı istenmeyen durumlarla karşılaşmamak için kendi kendine lavman uygulamasının hasta için gerekliliği iyi değerlendirilmeli, işlem öncesi kişiye yeterli eğitim verilerek tedavi devam ettiği sürece takibine devam edilmeli ve gereksiz uygulamaların önüne geçilmelidir.
An Extraordinary Case of Rectosigmoid Ischemia and Perforation Related to Enema Administration
Self-administration of enema has been widely accepted by many patients as it is a method which is easy to learn and apply. An 85-year-old female patient, who had no history of known disease was brought to the emergency department with abdominal pain which was pronounced in the lower quadrants initially but had spread to the whole abdomen within the last few hours and had become more severe. It was learnt that the patient had been occasionally self-administrating enema with a garden hose and tap water because of constipation for the last 4 years and had increased the rate of enema administration recently as she had not achieved enough relief. The patient was diagnosed with hollow organ perforation and ischemia and perforation were detected in the rectosigmoid colon during surgery. The patient died of multiple organ failure in the post-operative period. To avoid complications related to this technique, the necessity of self-administration of enema for the patients should be carefully evaluated. The patients should be followed-up as long as their treatments continue with adequate training before enema administration and unnecessary administrations must be prevented.
