TİROİDEKTOMİDE İNTRAOPERATİF NÖROMONİTORİZASYON KULLANIM DENEYİMİMİZ

Giriş: Multinodüler guatr ya da tiroid kanseri nedeniyle bilateral total tiroidektomi (TT)ülkemizde ve tüm dünyada sıklıkla uygulanan operasyonlardan biridir. TT’nin en önemli komplikasyonlarından biri nervus laringeus inferior (NLI) sinirinin geçici ya da kalıcı olarak hasarlanmasıdır. Bu komplikasyonun azaltılması amacıyla TT ameliyatlarısırasında son yıllarda intraoperatif nöromoitorizasyon (İONM) kullanılmaktadır. Bu çalışmada özel bir merkezde TT operasyonunda İONM uygulaması deneyiminin paylaşılması amaçlanmıştır. Gereç ve Yöntem: 2016-2020 tarihleri arasında hastanemiz genel cerrahi kliniğinde TT sırasında İONM uygulanan hastalar çalışmaya dahil edildi. Çalışmaya dahil edilen hastaların dosyaları retrospektif olarak incelendi. Hastaların yaş, cinsiyet, TT endikasyonları, postoperatif patolojik bulguları kayıt altına alındı. İONM bulgularında anormallik olan ve postoperatif dönemde NLİ hasarlanması düşünülen hastalara kord vokal muayenesi yapıldı. Bulgular oransal olarak değerlendirildi. Bulgular: Çalışma döneminde hastanemiz genel cerrahi kliniğinde 104 hastaya İONM kullanılarak TT uygulandı. Hastaların yaş ortalaması 51,0 (19-84) ve 85’i kadın (%81,7)’dı. Hastaların 34’ünd etiroid kanseri saptandı(%32,6). Bunların 30’u (%88,2) papiller, 1’si folliküler, 2’si medüller, 1’i anaplastik ve 1 tanesi de mixt karsinom olarak saptandı. Papiller karsinomların 18 tanesi mikrokarsinom (%60,0) olarak görülürken 11 tanesi (%36,6)’sımultisentik karsinom olarak görüldü. Hastaların hiçbirinde preoperatif NLİ hasarı yoktu. İONM sırasında 3 hastada (%2,8) tek taraflı sinyal kaybı oldu. Bu hastalarda NLI’de anatomik bütünlük bozulmamıştı. Sinyal kaybının geri gelmesi amacıyla 30 dakika beklendi ve sinyal geri gelmediği için operasyona son verildi. Bu hastaların 1’imultinoduler guatr, 1’i Graves, 1’i papiller karsinom nedeniyle opere edilmişlerdi. Postoperatif takipde hastaların her üçünde de klinik olarak ses değişikliği saptanmadı. Kord vokal muayenesinde tek taraflı paralizi görüldü. 3 ay takip sonunda her 3 hastada da kord vokal hareketleri geri geldi. Seride hiçbir hastada postoperatif kord vokal paralizisi olmadı. Sonuç: Sonuç olarak İONM total tiroidektomi sırasında cerrahın NLİ’u görüp korumasına rağmen oluşabilecek termal ya da traksiyon hasarlarını farketmemizi ve çift taraflı sinir hasarı oluşmasını engeller.

OUR EXPERIENCE IN THE USING OF INTRAOPERATIVE NEUROMONITORIZATION IN TOTAL THYROIDECTOMY

Introduction:Bilateral total thyroidectomy (TT) due to multinodular goiter or thyroid cancer is one of the most frequently performed operations in our country and all over the world. One of the most important complications of TT is the temporary or permanent damage of the nerve laryngeal inferior (NLI) nerve.In order to reduce this complication, intraoerative neuromotorization (IONM) has been used during TT operations in recent years. In this study, weaimed to share the experience of IONM application in TT operation in a private hospital. Material and Method: Patients who were performed IONM during TT in the general surgery clinic of our hospital between 2016-2020 were included in the study.The files of the patients included in the study were analyzed retrospectively. Age, gender, TT indications and postoperative pathological findings of the patients were recorded. Cord vocal examination was performed in patients with abnormal IONM findings during surgery and suspected NLI injury in the postoperative period. Results:During the study period, 104 patients underwent TT with using IONM in the general surgery clinic of our hospital.The mean age of the patients was 51.0 (19-84) and 85 (81.7%) of the patients were female.Thyroid cancer was detected in 29 patients (32,6%).Of these, 30 (88.2%) were papillary, 1 was follicular, 2 were medullary, 1 was anaplastic and 1 was mixed carcinoma.While 18 of the papillary carcinomas were microcarcinoma (60,0%), 11 (36.6%) were multicentic carcinoma. None of the patients had preoperative NLI injury.Unilateral signal loss occurred in 3 patients (2.8%) during IONM. In these patients, anatomic integrity was not impaired in NLI. The signal was waited for 30 minutes to recover and the operation was terminated because the signal did not return. The operation endication was multinodullary goiter in 1 patients, papillary carcinoma in one patient and Graves disease in 1 patient. No voice change was detected clinically in any patients during postoperative follow-up.Cord vocal examination revealed unilateral paralysis. At 3 months follow-up, cord vocal movements returned to both patients.None of the patients had postoperative cord vocal paralysis. Conclusion:In conclusion, IONM alerts us to recognize thermal or traction damages during bilateral TT, although the surgeon sees and protects the NLI and IONM prevents the development of bilateral nerve damage.
