PRİMER PROSTAT BİYOPSİSİ SONRASI RE-BİYOPSİ ÖNERİLEN HASTALARDA ANKSİYETE VE DEPRESYONUN DEĞERLENDİRİLMESİ

Giriş: Primer transrektal ultasonografi eşliğinde prostat biyopsisi (TRUS-Bx) sonrası şüpheli kanser ön tanısı ile repeat biyopsi (re-Bx) önerilen hastalardaki anksiyete ve depresyonu değerlendirmek Gereç ve Yöntem: Haziran 2017- Haziran 2018 tarihleri arasında prostat spesifik antijen (PSA) yüksekliği ve/veya anormal rektal muayene bulgusu nedeniyle TRUS-Bx yapılan hastalar çalışmaya dahil edildi. TRUS-BX öncesi, hemen sonrası ve 1. ay sonunda patoloji raporu ile birlikte sorgulama formları (Durumluk ve Sürekli Kaygı Ölçeği (StateTraitAnxiety Inventory (STAI) formu), Hastane Anksiyete ve Depresyon Ölçeği (Hospital Anxiety Depression Scale (HADS) ve algılanan stres ölçeği (ASÖ-14)) ile hastaların anksiyete ve depresyon durumları değerlendirildi. Hastalar histopatolojik inceleme sonunda Atypical Small Acinar Proliferation (ASAP) tanısı konulup re-biyopsi önerilen (Grup 1) ve benign patoloji gözlenip re-Bx önerilmeyenler (Grup 2) olarak 2 gruba ayrıldı. İki grup yaş, PSA, prostat volumü, anestezi şekli ve sorgulama formu sonuçları açısından karşılaştırıldı. Bulgular: TRUS-Bx alınan 102 hasta incelendi. Malign patoloji tanısı olan 31 hasta çalışma dışı bırakılarak toplam 71 hasta çalışmaya dahil edildi. Grup 1 de 19 Grup 2 de 52 hasta vardı. Gruplar arasında yaş, PSA, prostat volümü ve uygulanan anestezi şekli açısından istatistiksel anlamlı farklılık izlenmedi (p>0,05). Sorgulama formlarına baktığımızda, STAI TX-1 ve TX-2, HADS-A ve HADS-D ve ASÖ-14 verilerinin TRUS-BX öncesi, hemen sonrası ve 1. ay sonundaki verilerin gruplar arası benzer olduğu gözlendi. Sonuç: Primer prostat biyopsi işleminden sonra kanser şüphesi bulunan hastalara önerilen re-Bx işlemi hastalarda ek psikolojik stres yaratmamaktadır.

EVALUATION OF ANXIETY AND DEPRESSION IN PATIENTS RECOMMENDED REBIOPSY AFTER PRIMARY PROSTATE BIOPSY

Introduction: We aimed to evaluate anxiety and depression in patients undergoing repeat biopsy (re-Bx) with suspected cancer prediction following primary transrectal ultrasonography-guided prostate biopsy (TRUS-Bx). Material and Method: Patients who underwent TRUS-Bx due to prostate specific antigen (PSA) elevation and / or abnormal rectal examination findings between June 2017 and June 2018 were included in the study. The patients' anxiety and depression status were evaluated with a questionnaire (STAI, PSS and HADS) before, immediate after and at the end of the first month along with pathology report after TRUS-BX. The patients were divided into two groups as patients diagnosed with Atypical Small Acinar Proliferation (ASAP) at the end of histopathological examination and recommended re-biopsy (Group 1) and with benign pathology and re-Bx was not recommended (Group 2). The groups were compared in terms of age, PSA, prostate volume, anesthesia type and scores of questionaries . Results: one hundred and two patients who received TRUS-Bx were investigated. Thirty-one patients with malignant pathology were excluded from the study and 71 patients were included in the study. There were 19 patients in Group 1 and 52 in Group 2. No statistically significant difference was found between the groups in terms of age, PSA, prostate volume and anesthesia type (p> 0.05). We observed that the data of STAI TX-1 and TX-2, HADS-A and HADS-D and PSS-14 data were similar between the groups at the end of TRUS-BX, immediately after and at the end of the first month. Conclusions: To recommend re-Bx procedure for patients with suspected cancer after primary prostate biopsy does not cause additional psychological stres.
