Normal Gebe Populasyonda İntrakardiak Ekojenik Foküs Sıklığı ve Anomaliler ile İlişkisi

Amaç: Hastanemizde takip edilen olgularda fetal intrakardiak ekojenik foküs sıklığını belirlemek ve olası anomaliler arasındaki ilişkiyi araştırmaktır. Metod: Ocak-2010 ile Ocak-2014 tarihleri arasında hastanemizde takip edilen ve anomali taraması için radyoloji bölümüne refere edilen 1542 gebenin kayıtları retrospektif olarak incelendi. Tüm olgulara 20. haftada ayrıntılı fetal değerlerndirme yapılmıştır. Ultrasonografik incelemede İKEF tespit edilen olgularda, ekojenik foküsün yerleşim yeri ve boyutu, kardiyak ek anomaliler kaydedilmiştir. Trizomi 21'in potansiyel ekstrakardiak bulguları açısından kranium, ekstremiteler, böbrek ve barsaklarlar dikkatlice değerlendirilmiştir. İntrakardiak ekojenik foküs saptanan fetüslerin doğum sonu değerlendirmeleri bir çocuk hekimi tarafından yapılmıştır. Bulgular: 1542 olgunun 38 tanesinde İKEF tespit edildi. İKEF görülme sıklığını %2.46 olarak hesaplandı. Ortalama gebe yaşı 28,21 (22-35) idi. Olguların 36 tanesinde İKEF sol ventrikül içerisinde ve ikisinde sağ ventrikül içerisinde gözlendi. Bir olguda sol ventrikül içerisinde iki adet İKEF mevcuttu. İKEF boyutu 2-3.9 mm arasında değişiyordu ortalama boyut 2.45 mm idi. 38 olgunun 24 tanesinde İKEF birinci gebelikte (%63), 12 tanesinde ikinci gebelikte (%31) ve 2 tanesinde (6) ise üçüncü gebelikte görülmüştü. İKEF'li 38 olgunun ikisi IVF gebelik olup bunlardan birisi ikiz diğeri ise üçüz gebelikti. İki eşinde ve üçüzdeki diğer iki fetüste İKEF izlenmedi. İKEF saptanan fetüslerden sadece birisinde sol koroid pleksusta 8x6 mm'lik basit kist saptandı ve bu kist takiplerde kayboldu. İKEF'li olguların doğum sonu, neonatal dönem ve 2. ayda yapılan fizik muayenelerinde anomali düşündürecek patoloji saptanmadı. Sonuç: ICEF çoğu kez önemli bir problemin parçası olarak speküle edilmekte ve özellikle gebelere anksiyete getirmektedir. Fakat çalışmamızda İKEF-Trizomi birlikteliğine ait kanıt bulamadık. Özellikle 35 yaş altı, üçlü tarama testleri negatif ve ultrasonografik olarak ek anomali izlenmeyen gebelerde izole ekojenik kardiak foküslerde amniosentez gerekli olmayabilir.

The Relationship between the Frequency of Intracardiac Echogenic Focus and Anomalies in Normal Pregnancies

Backgrounds: To determine the frequency of intracardiac echogenic focus (ICEF) in a normal pregnant population and to investigate the relationship with anomalies. Methods: A retrospective examination was made of 1542 pregnancy records where the patients were monitored at our hospital and referred to the Radiology Department for an anomaly scan between January 2010 and January 2014. A detailed fetal evaluation was made at 20 weeks for all patients. In patients determined with ICEF on ultrasonographic examination, a record was made of the echogenic focus location and size and additional cardiac anomalies. In respect of potential extracardiac findings of trisomy 21, the cranium, extremities, kidneys and intestines were carefully evaluated. Postnatal evaluations were made of fetuses determined with ICEF. Results: Of 1542 cases, ICEF was determined in 38 (2.46%). The mean age of the pregnant patients was 28.21 years (range, 22-35 years). ICEF was observed in the left ventricle in 36 cases and the right ventricle in 2 cases. In 1 case, there were 2 ICEF in left ventricle. The mean size of the ICEF was 2.45mm (range, 2 – 3.9mm). Of the 38 cases, the ICEF was observed in the first pregnancy in 24 cases (63%), the second pregnancy in 12 (31%) and the third pregnancy in 2 (6%). Of the 38 cases with ICEF, 2 were IVF pregnancies; one delivered twins and the other, triplets. ICEF was not observed in the other twin or in the other two of the triplets. In only 1 of the fetuses determined with ICEF, a simple cyst of 8x6mm was determined in the left choroid plexus, which spontaneously disappeared during follow-up. No pathologies which could be considered anomalies were determined in the physical examinations made of the ICEF cases at birth, in the neonatal period and in the 2nd month. Conclusions: Even though ICEF is considered as an indicator of trisomy anomalies, it may be seen in pregnant patients aged below 35 years, with negative triple scan tests and no additional anomalies observed ultrasonographically.
