Renal transplantasyon yapılan 388 erişkin hastanın analizi: Ege Üniversitesi sonuçları

Amaç: Bu çalışmada, merkezimizde bu güne dek izlenen erişkin renal transplantasyon hastalarının demografik analizinin sunulması, merkezimizin kadaverik ve canlı vericili renal transplantasyon deneyiminin aktarılması amaçlanmaktadır. Yöntem: Ege Üniversitesi Organ Nakli Uygulama ve Araştırma Merkezi'nde 12 Ekim 1987 ile 31 Aralık 2000 tarihleri arasında 512 erişkin renal transplantasyonlu hasta izlenmiş, bunlardan 388'ine merkezimizde renal transplantasyon yapılmıştır. Olguların greft ve hasta sağ kalım oranları Kaplan Meier analizi ile değerlendirilmiştir. Bulgular: Renal transplantasyonlu 388 erişkin hastanın 237'si erkek, 151'i kadın olup; yaş ortalaması 35±10 (17-64) yıldır. Transplant greftle ortalama yaşam süresi 43±33 (33-136) aydır. Bu hastaların 318'i canlı vericili renal transplantasyonlu, 70 tanesi kadaverik vericili renal transplantasyonludur. Kadaverik vericili renal transplantasyonlu 70 hastanın 54'ü 1997-1999 yıllarında opere edilmiştir. Kadavra vericili 70 olgunun 1, 3 ve 5 yıllık hasta sağ kalım oranları sırasıyla % 98, 88 ve 84 bulunurken, 1994 sonrası yapılan 213 canlı vericili renal transplantasyon olgusunun 1, 3 ve 5 yıllık hasta sağ kalım oranı % 95, 93 ve 89 bulunmuştur. Bir, 3 ve 5 yıllık greft sağ kalım oranları kadaverik vericili renal transplantasyon için % 95, 85 ve 76 iken, bu sayı canlı vericili renal transplantasyon için % 92, 82 ve 70 bulunmuştur. Sonuç: Özellikle 1994 yılı sonrasında kadaverik vericili ve canlı vericili renal transplantasyonda greft ve hasta sağ kalımı açısından önemli bir fark saptanmamıştır. Son yıllarda artan kadaverik renal transplantasyon sayısı ve elde edilen başarılı sonuçlar ümit vericidir.

The analysis of 388 adult patients on whom renal transplantation was performed: Ege University results

Objective: The aim of this study was to make an analysis of our transplantation cases and to present our experience on this field. Methods: In the Ege University Organ Transplantation and Research Center, 512 renal transplanted adult patients were followed between 12 Oct 1987 to 31 Dec 2000. Three hundred and eighty-eight of these patients were transplanted in our center. The graft and survival of the cases were analyzed with Kaplan Meier analysis method. Results: 237 of the patients were male and 151 were female, their mean age was 35±10 (range 17-64 years) years. Mean survival time with transplanted graft was 43±33 (33-136) months. 318 of these patients had live renal donors and 70 had cadaveric renal donors. 54 of 70 cadaveric renal donors patients were operated in the last three years (1997-1999). One, 3 and 5 years survival ratios of cadaveric renal donor patients were 98%, 88% and 84% respectively. On the other hand, the survival ratios of 213 live renal donor patients operated after 1994 were 95%, 93% and 89%. One, 3 and 5 years graft survival ratios were 95%, 85% and 76% in cadaveric renal donor patients and were 92%, 82% and 70% in live renal donor patients. Conclusion: There was no difference in comparison of the transplantation success of live renal donor and cadaveric renal donor patients. Renal transplantation frequency has being increasing since 1994 and the progress in this field is encouraging.
