Prevention of lung injury occured during cardiopulmonary bypass

Objective: Cardiopulmonary bypass-induced pulmonary injury is still an important problem. In this experimental study we investigated the effectiveness of pre-operative and intra-operative pentoxyfilline and aprotinin administrations to reduce the pulmonary injury. Methods: Eighteen healthy dogs were divided to three equal groups; control group (group I, n=6), pentoxyfilline group (group II, n=6), and aprotinin group (group III, n=6). Standard cardiopulmonary bypass was performed to all subjects under general anesthesia. Blood and tissue samples were taken from the right upper pulmonary veins and the right upper lobes preoperatively, and after the termination of cardiopulmonary bypass. Histopathological examinations were made under light microscope. In biochemical studies, IL-6, TNF-alpha, and GSH-PX levels were measured in the sera of the blood samples. Results: Aprotinin successfully reduced the CPB-induced inflammatory cell infiltration and alveolar septal defect when compared with the control group. No statistically significant difference was found when pentoxyfilline group was compared to the control group. Both aprotinin and pentoxyfilline significantly reduced the IL-6 levels when compared to the control group. Conclusion: In the aprotinin group cardiopulmonary bypass induced pulmonary injury was seriously reduced both histopathologically and biochemically, while pentoxyfilline reduced only biochemically the pulmonary injury.
