Bruselloz şüpheli olgularda Rose Bengal, Standart Tüp Aglütinas- yon ve Brucellacapt sonuçlarının değerlendirilmesi

Bu çalışmada, bruselloz şüpheli hastalarda Rose Bengal tarama testi sonuçları, Standart Tüp Aglütinasyon (STA) ve Brucellacapt titreleri geriye dönük olarak değerlendirilerek bu testlerin bruselloz tanısındaki etkinliğinin araştırılması ve şüpheli hastalarda bruselloz sıklığının belirlenmesi amaçlanmıştır. Gereç ve yöntem: Bruselloz şüpheli toplam 1060 hastanın serum örneği çalışmaya dahil edilmiştir. Serum örneklerine öncelikle tarama amaçlı Rose Bengal lam aglütinasyon testi yapılmıştır. Rose Bengal tarama testi pozitif bulunan örneklere Standart Tüp Aglütinasyon ve Brucellacapt uygulanmıştır. Bulgular: Toplam 1060 serum örneğinin 115'inde (%10,8) Rose Bengal tarama testi pozitiftir. Standart Tüp Aglütinasyon testi, tarama testi pozitif olan örneklerin 88'inde (%76,5) 1/160 ve üzeri titrede pozitif bulunmuştur. Bu örneklerin Brucellacapt sonuçları değerlendirildiğinde ise, 86 (%74,8) örnekte 1/320 ve üzeri titrede aglütinasyon saptanmıştır. Her üç testin de pozitiflik oranı kadınlarda erkeklerden daha düşük bulunmuş ve bu istatistiksel olarak anlamlı kabul edilmiştir. Yaş ile seropozitiflik arasında anlamlı bir ilişki bulunamamıştır. Hastaların başvuru sırasındaki en sık şikayetleri eklem ağrısı, kas ağrısı, ateş, bel ağrısı, kilo kaybı ve halsizlik olarak belirlenmiştir. Sonuç: Rose Bengal ve Standart Tüp Aglütinasyon testleri brusellozun serolojik tanısında hala geçerliliğini korumaktadırlar. Brucellacapt testi de blokan antikorları tespit etmesi nedeniyle tanıda kullanılabilecek etkin bir yöntemdir

In this study, it was aimed to investigate the effectiveness of Rose Bengal, Standard Tube Agglutination and Brucellacapt methods in diagnosis of brucellosis by evaluating the results of Rose Bengal screening test and titers of Standard Tube Agglutination and Brucellacapt, retrospectively and determine seroprevalence of brucellosis in suspected patients. Material and methods: A total of 1060 serum samples of brucellosis suspected patients were included to this study. All serum samples were first screened by Rose Bengal lam agglutination test. Serum samples which were found to be positive by Rose Bengal screening test were further analysed by Standard Tube Agglutination test and Brucellacapt. Results: Rose Bengal screening tests were positive in 115 (10.8%) of 1060 serum samples. Standard Tube Agglutination tests were found to be positive for 1/160 and higher titers in 88 (76.5%) of Rose Bengal screen- ing test positive serum samples. When Brucellacapt results were evaluated, titers of 1/320 and above were detected in 86 (74.8%) of screening test positive serum samples. The seropositivity rates of these three tests were found to be lower in women than men and this was detected as istatistically significant. No significant association was found between age and seropositivity. The most frequent complaints of patients at presentation were determined as arthralgia, myalgia, fever, backache, weight loss and fatigue. Conclusion: Rose Bengal and Standard Tube Agglutination tests are still effective methods for serodiagnosis of brucellosis. Also, Brucellacapt is an efficient method for diagnosis with an advantage of detecting blocking antibodies
