Arnold-Chiari malformasyonu: Literatür eşliğinde vaka tartışması

Amaç: Arnold-Chiari sendromu oksipital ağrı, tinnitus, üst ekstremitede ağrı gibi sık rastlanan non-spesifik semptomlara ve tek taraflı hipoglossal sinir ve vokal kord paralizisi, hemifasiyal spazm, nistagmus, trigeminal ve glossofaringeal nevralji ve her iki kulakta işitme kaybı gibi spesifik semptomlara neden olmaktadır. Bu çalışmada 2 olgu ve literatür eşliğinde Arnold-Chiari sendromunu tartıştık. Olgu sunumu: Bu çalışmada CM tip 1 ile direkt ilişkili olmayan non spesifik semptomlarla Beyin ve Sinir Cerrahi polikliniğine başvuran 2 olguyu literatür eşliğinde sunduk. Sonuç: Arnold-Chiari olguları, nörolojik anomaliler olmaksızın başka nedenle açıklanamayan senkop ve drop ataklar geçirebilmekte ve daha da önemlisi ani kardiyorespiratuar arrest sonucu ölüm görülebilmektedir. Arnold-Chiari sendromu ayırıcı tanı olarak bu tip hastalarda düşünülmelidir.

Arnold-chiari malformation: Case discussion with literature

Objective: Arnold-Chiari syndrome cause non-specific symptoms such as occipital pain, tinnitus, upper extremity pain and specific symptoms such as unilateral hypoglossal nerve and vocal cord paralysis, hemifacial spasm, nystagmus, trigeminal and glossofaringeal neuralgia and loss of hearing in both ears. In this study, Arnold-Chiari syndrome in two cases and review of the literature is discussed. Case report: In this study, two cases with review of the literature is discussed Conclusion: Arnold-Chiari patients without neurological abnormalities may have unexplained syncope and dropped attack and more importantly sudden death due to cardiorespiratory arrest can be seen. Arnold-Chiari syndrome should consider as a differential diagnosis in this patients.
