Sağlıklı Yetişkin Erkeklerde Çeşitli Vücut Kompozisyon Parametreleri ve Arteryel Sertlik Arasındaki İlişki

Amaç: Obezite ve arteryel sertlik, kardiyovasküler hastalık riskini arttırmaktadır. Antropometrik ölçümler ve çeşitli vücut kompozisyon parametrelerinin arteryel sertlik ile ilişkili olduğu bilinmektedir. Ancak antropometrik ölçümlerden viseral yağlanmanın arteryel sertlik ile ilişkisi yeterince bilinmemektedir. Bu çalışmada, total vücut yağ kitlesi ve viseral yağlanma ile arteryel sertlik arasındaki ilişkiyi araştırıldı. Gereç ve Yöntem: Çalışmaya sağlıklı 52 erkek birey alındı. Tüm olguların kan basıncı, ayak-kol basınç indeksi ve bel/kalça oranı standart protokole göre ölçüldü. Viseral yağ kitlesi, total vücut yağ kitlesi, bazal metabolizma hızı, metabolik yaş ve beden kitle indeksini içeren vücut kompozisyon parametreleri Tanita BC-418 vücut kompozisyon analizörü ile biyoelektriksel empedans yöntemiyle ölçüldü. Arteryel sertliği değerlendirme için nabız dalga hızı ve augmentasyon indeksi, tensioMedTM arteriyografi cihazı ile ölçüldü. Bulgular: Çalışmaya alınan olguların yaş ortalaması 40 ± 12 yıl idi. Nabız dalga hızı, 9.14 ± 1.55 m/sn ve augmentasyon indeksi, %16.66 ± 9.53'di. Total vücut ve viseral yağ kitlesi sırayla % 23 ± 7 ve 9 ± 4 olarak ölçüldü. Viseral yağ kitlesi; yaş, sistolik kan basıncı, BKİ, nabız dalga hızı ve arteryel yaş ile pozitif korelasyon göstermektedir. Çoklu regresyon analizinde, sadece viseral yağ kitlesi nabız dalga hızı ile bağımsız olarak ilişki göstermekteydi (ß=0.327, p=0.014). Sonuç: Viseral yağlanma, sağlıklı yetişkin erkeklerde arteryel sertliği göstermede, total vücut yağ kitlesinden daha iyi bir öngördürücüdür.

The Relationship between Various Measures of Body Composition and Arterial Stiffness in Healthy Male Individuals

Obesity and arterial stiffness are associated with the increased risk of cardiovascular disease. It is known that anthropometric measurements and several parameters of body composition is related with arterial stiffness. However, association of visceral fat - an anthropometric measurement - with arterial stiffness is sufficiently unknown. The aim of this study is to examine the possible relationship between various measures of body composition and arterial stiffness. Material and Method: 52 healthy males were included in the study. In all participants, blood pressure, ankle-brachial pressure index, and waist/hip ratio were measured according to standard protocol.Several body composition parameters were calculated by bioelectrical impedance analysis using Tanita BC-418 segmental body composition analyzer. To evaluate arterial stiffness, arterial age, pulse wave velocity (PWV), and augmentation index, were calculated by tensioMedTM Arteriograph. Results: The mean age of the subjects was 40 ± 12 years (in the range of 18 to 62 years). The PWV was 9.14 ± 1.55 m/sn and augmentation index was 16.66 ± 9.53 %. Total and visceral fat mass were measured as 23 ± 7% and 9±4% respectively. Visceral fat mass was positively correlated with age, systolic blood pressure, body mass index, PWV and arterial age. Multiple linear regression analysis showed that the increased PWV was independently associated with visceral fat mass (ß=0.327, p=0.014). Conclusion: In healthy adult males, visceral fat is a better predictor for arterial stiffness compared with total body fat mass.
