Önlenemeyen Halk Sağlığı Sorunu: Çocuklarda Koroziv Madde İçimi
Amaç: Koroziv maddelerin kaza ile içilmesi ülkemizde sıklıkla karşılaşılan bir halk sağlığı sorunudur. Bu çalışmada kliniğimize koroziv madde içme yakınmasıyla getirilen çocuk hastaların irdelenmesi amaçlandı. Gereç ve Yöntem: Son iki yıl içerisinde koroziv madde içme yakınmasıyla getirilen toplam 126 çocuk hasta çalışmaya alındı. Olguların dosya kayıt-ları geriye dönük olarak incelendi. Bulgular: Hastaların ortalama yaşı 46.48+47.93 ay olup erkek/ kız oranı 1.2 idi. İçilen koroziv maddelerin çoğunluğunu alkaliler oluşturmakta ve bunların büyük kısmı da markasız çamaşır sularıydı. Endoskopik değerlendirmede %46.8’inde gastrointestinal sistemde herhangi bir patoloji saptan-mazken geriye kalan olguların büyük çoğunluğunu evre-1 ve 2a özofajitli olgular oluşturmaktaydı. %17.4 olguda evre-2b ve üzeri özofajit saptandı. Kireç-çöz, yağ-çöz, tuz ruhu gibi koroziv maddelerin mukoza üzerinde ağır yanık yapıcı etkilerinin daha fazla olduğu görüldü. Sonuç: Kolay elde edilen koroziv maddelerin içimi önemli morbiditelere neden olabilir. Bu konuda ailelerin bilinçlendirilmesi önemlidir.
An Unavoidable Public Health Problem: Ingestion of Corrosive Substance in Children
Objective: Accidentally ingestion of corrosive substance is an often encountered public health problem in our country. The aim of this study was to evaluate the pediatric patients admitted due to ingestion of corrosive substance to our clinic. Material and Method: In total 126 pediatric patients were included in this study who admitted to our hospital with ingestion of corrosive substance in the last two years. Medical records of those patients were analysed retrospectively. Results: The mean age of the patients was 46.48 ±47.93 months and male/female ratio was 1.2. Majority of ingested corrosive substances were alkalis and mostly of them were unbranded bleaches. According to endoscopical evaluation no pathology was detected in 46.8% of patients, and the majority of remaining cases had grade 1 and 2a esophagitis. 17.4% of patients had grade 2b and higher grades of esophagitis. It is observed that corrosive substances such as lime-solve, oil-solve, hydrochloric acid were more caustic on mucous membranes. Conclusion: Ingestion of easily accessible corrosive substances can cause significant morbidities. It is important to raise the awareness of parents about this issue.
