Böbrek Nakli Yapılan Hastalarda İdrar Yolu Enfeksiyonları: Sıklığı, Etkenler ve Risk Faktörleri

ÖZET Amaç: Son dönem böbrek yetmezliğinin seçilmiş hastalardaki en başarılı tedavi yöntemi böbrek naklidir. Böbrek nakli sonrası alıcıların % 75’inde ilk yıl içinde enfeksiyonlarla karşılaşılmaktadır. Biz hastanemizde yeni kurulmuş bir nakil merkezinde böbrek nakli yapılmış hastalarda idrar yolu enfeksiyonu (İYE) sıklığını, etkenleri ve risk faktörlerini tespit etmeyi amaçladık. Gereç ve Yöntem: Çalışmaya böbrek nakli yapılan 42 hasta alındı. Hastaların yaş, cinsiyet gibi demografik özellikleri, kronik böbrek yetmezliği (KBY) etyolojileri, transplantasyon öncesi diyaliz tipleri ve süresi, aldıkları immünsupresif tedavi, nakil esnasında yatış süreleri, donörün kadavra veya canlı olup olmadığı, üreyen etkenler retrospektif olarak incelenerek veriler kaydedildi. Bulgular: Kırk iki hastanın 13 (%31)’ ünde transplantasyon yapıldıktan sonra ilk 6 ay içerisinde idrar yolu enfeksiyonu, bu hastaların 4’ünde de en az iki enfeksiyon atağı olduğu saptandı. Etkenler arasında Escherichia coli ve Klebsiella pneumonia ilk iki sıradaydı. İYE olan ve olmayan iki grup karşılaştırıldığında nakil esnasında yatış süreleri açısından fark anlamlı bulunmuş, sürenin uzunluğu ile enfeksiyon gelişiminin arttığı tespit edilmiştir (p=0.001) Sonuç: Böbrek transplantasyonu yapılan hastalarda problem teşkil ettiğinden İYE mutlaka değerlendirilmelidir. Risk faktörleri değerlendirildiğinde hastalarımız arasında nakil esnasında yatış süresi İYE riskini arttırmıştır İYE riskini azaltmak için hastaların yatış süresini en aza indirmek gerekir.

Urinary Tract Infections in Renal Transplant Patients: Prevelance, Agents and Risk Factors

Objective: Infection are encountered in 75% of patients who underwent renal transplantation which is the most successful method of treatment among selected patients with end-stage renal failure in the first year. We wanted to determine the infection ingredients and risk factors of urinary tract infection (UTI) after renal transplantation by patients in a newly-established transplant center in our hospital. Material and Method: Fourty-two patients were included to this study. Patients age, gender, chronic renal failure etiology, pretransplant dialysis types and duration, received immunosuppressive therapy, duration of hospitalization during transplantation, whether cadaveric or living donor, reproducing infectious agents were studied retrospectively and data were recorded. Results: Of 42 patients, in 12 (31%) UTI was observed in the first 6 months after renal transplantation. The four of these patients revealed at least two episodes of infection. Escherichia coli and Klebsiella pneumonia were found to be the most isolated microorganisms. When comparing the two groups with and without UTI, length of stay during transplantation was significantly different between the two groups (p=0.001) Conclusion: UTI in renal transplant patients is important. In patients who underwent kidney transplantation in our hospital, length of stay during transplantation is an increased risk of UTI. To reduce the risk of urinary tract infection, duration of hospitalization should be minimized.
