Mide mukozasındaki eozinofil yoğunluğu mevsimlere göre değişiklik gösteriyor mu?
Çalışmamızda, mide mukozasındaki eozinofillerin "dönem dönem" arttığı gözlemimizden hareketle, yoğunluğun mevsimlerarası farklılık gösterip göstermediği ve eşlik eden patolojilerle ilişkisi araştırılmıştır. Son bir yıl içerisinde endoskopik biyopsileri değerlendirilen erişkin olgular arasından, eozinofil artışına yolaçabilecek hastalığı olanlar dışlanmak suretiyle, her ayın ilk 10 olgusu seçilerek, 12 "ay grubu" na ayrılmış, 120 olguluk bir seri oluşturuldu. Antrum ve korpustan alınan 240 biyopsi örneğinde ardışık 10 alan sayılarak ortalaması alındı ve bir büyük büyütme alanındaki ortalama eozinofil yoğunluğu (OEY) Eo/BBA olarak kaydedildi. Aylar ve mevsimler arasındaki fark ve OEY ile yaş, cinsiyet ve eşlik eden gastrit ve H.pilori (HP) enfeksiyonu arasındaki ilişki T-testi ve nonparametrik testlerle değerlendirildi.Kadın/erkek oranı 1,4; yaş ortalaması 48±16 (D:20-83 yıl); OEY: 0,80±0,69 Eo/BBA (0-3,35 Eo/BBA) idi. Does the gastric mucosal eosinophil concentration show seasonal variations?
Resting on our observation that gastric mucosal eosinophil density increases from "time to time", we sought for a possible seasonal variation and relationship with accompanying pathologies. Among the biopsies evaluated during the previous year, twelve "month group" were arranged each including first 10 patients' biopsies of every month, excluding the cases having disorders that may lead to elevated eosinophil counts. In a total of 240 antral and corporal biopsies of 120 adult patients, 10 consecutive areas were counted and mean eosinophil density (MED) in one high power field was determined as Eo/HPF. The difference between MED's of months and seasons and relationship with age, gender, gastritis and H.pylori (HP) infection were evaluated by T-test and nonparametric tests. Female/male ratio was 1.4; mean age was 48±16 (R: 20-83 yr) and MED was: 0.80±0.69 Eo/HPF (0-3.35 Eo/HPF). The difference between antral and corporal eosinophil densities were insignificant. Although no significant difference was observed between seasons and the pollen season, March-August period vs off-season, MED's appeared to increase in July-A ugust-September period (p <0.05). The highest and lowest MED's were observed between Semptember and December, the months harboring the highest and lowest number of patients with HP infection and gastritis, as well (1.40+1.0 Eo/HPF vs 0.45±0.51 Eo/HPF, p<0.05). We concluded that MED's can present monthly variations although no relationship was observed with pollen season. It seems to be affected from the presence of HP infection and gastritis. So, HP infection, especially in endemic reagions, should be kept in mind when evaluating gastric mucosal eosinophil density.
