Karaciğer sirozlu hastalarda tiroid hormon düzeyleri ve mortalite ile ilişkileri

Karaciğer parankiminin tiroid hormon metabolizmasında major yollardan biri olduğu bilinmektedir. Serum thyroid hormone levels in patients with hepatic cirrhosis and relationship with mortality

Hepatic parenchymal tissue is known to be one of major sites of thyroid hormone metabolism. In this study, serum levels of serum triiodothyronine (T3), thyroxine (T4), free T3 (FT3), free T4 (FT4), and thyrotropin (TSH) were determined by radioimmunoassay in clinically euthyroid 35 patients with hepatic cirrhosis and 35 normal subjects. T3/T4 and FT3/FT4 ratios were calculated. 5 out of 35 patents with hepatic cirrhosis died of hepatic coma. Levels of serum T4, 73, FT4, and FT3 were lower than normal subjects (p<0.05, p<0.005, p<0.0005, and p<0.0005, respectively). FT3/FT4 ratio was decreased significantly in patients with hepatic cirrhosis (p<0.005). Patients who died of hepatic cirrhosis had significantly decreased T3 (p<0.05), T4 (p<0.005), and FT3 (p<0.05) values than alive hepatic cirrhosis patients. Serum albumin levels had significant correlations with serum T4 (7=0.69, p<0.005), FT4 (>=0.76, p<0.0001) and T3 (7=0.54, p<0.025) levels. This data suggests that, serum thyroid hormon levels significantly change in hepatic cirrhosis and the degree of changes is dependent on the severity of liver dysfunction. These findings may be helpful in assessing outcome of patients with hepatic cirrhosis.
