Etyolojide servikal vertebra metastazına bağlı gelişen radikülopati saptanan bir "fıçıdaki adam sendromu" olgusu

Alt ekstremite kasgücü normalken brakiyal diplejinin bulunması “fıçıdaki adam sendromu” (FAS) olarak bilinir ve genellikle prerolandik kortikal ve subkortikal alanlardaki bilateral supratentorial beyin lezyonlarına bağlı olarak geliştiği düşünülür. Literatürde FAS'a yol açan az sayıda infratentoriyel nedenli olgu bildirimleri bulunmaktadır ve bunlar genellikle damarsal patoloji ile ilişkilendirilmiştir. Olgu: 66 yaşında erkek olgu kollarındaki güçsüzlük ve ağrı yakınması ile başvurdu. Elektromyografi (EMG)'de C5, C6 da daha belirgin olmak üzere bilateral C5, C7 spinal radiküler tutuluş saptanan olgunun servikal MRG'sinde C5 vertebrospinal metastatik lezyon varlığı ortaya kondu. Yorum: Bu sunumda FAS kliniği ile prezente olan ve etyolojide servikal vertebra metastazına bağlı radikülopati saptanan bir olgu literatür eşliğinde tartışılmıştır.

A case presenting with "man in the barrel syndrome" whose etiological underlying cause is radiculopathy due to cervical vertebra metastasis

Brachial diplegia with normal findings of the legs, "man-in-the barrel" syndrome (MIBS), is generally thought to be caused by bilateral supratentorial brain lesions of the prerolandic cortical and subcortical area. There is only a few report in literature about MIBS regarding with infratentorial causes and they have usually vascular pathogenesis. Case: 66 years old men consulted for brachial dipleji and brachial pain. EMG showed bilaterally C5-C7 spinal radiculer involvement, especially in C5-C6 regions. Cervical MRG demonstrated C5 vertebrospinal metastasis. Conclusion: İn this report we describe a case presenting with MİBS whose etiological undelying cause is radiculopathy due to cervical vertebra metastasis.
