Erken evre mide kanserinde pilor koruyuculu mide rezeksiyonu (Olgu sunumu)

Günümüzde mide kanserinin standart tedavisi tümörün evresine göre değişmektedir. Genişletilmiş mide rezeksiyonu ve D2 lenf nodu diseksiyonu sıklıkla tercih edilen cerrahi yöntemlerden birisidir. Erken evre kanserlerde de bu tedavi uygulanabilmektedir. Erken evre mide kanserlerinin günümüzde daha sık tanınıyor olması cerrahi tedavi seçeneklerinin gözden geçirilmesine neden olmuştur. Bu yöntemlerden biri olan pilor koruyuculu mide rezeksiyonu (PKMR) ilk kez gastrik ülser cerrahisinde denenmiştir. PKMR uygulanan hastaların uzun dönem takibinde; ülser nüksünün ve dumpingin oldukça düşük sıklıkla ortaya çıktığı bildirilmiştir. Bu sonuçları nedeniyle PMKR'nun erken evre mide tümörlerinde de uygulanabileceği düşünülmektedir.

Pylorus-preserving gastrectomy for early gastric cancer (Case report)

Standard treatment of gastric cancer can alter according to the stage of the tumor. Extended gastrectomy with group 2 lymph node dissection (D2) is one of the treatment procedure of gastric cancer. The procedure is also valid for early gastric cancer. The surgical therapy is to be reviewed because early gastric cancer is more likely to be diagnosed today. Pylorus-preserving gastrectomy (PPG), as a surgical option has first been used for gastric ulcer surgery. Ulcer recurrence and dumping had been reported to be rare in the long term follow up of the patients with PPG. Because of this physiological benefits, we believe that PPG can be an appropriate surgical procedure for early gastric cancer as well.
