Çocuk yaş grubu idrar yolu enfeksiyonlarında Ege bölgesi antibiyotik direnci

Ülkemizde, bir çok yasal engele rağmen tüm enfeksiyonlarda olduğu gibi üriner enfeksiyonlarda antibiyotiklerin yanlış ve gereksiz kullanımı, antibiyotiklere direnç gelişmesine ve dirençli suşlarla yeni enfeksiyonların oluşmasına yol açmaktadır. Bu nedenle bölgesel antibiyotik direnci gözden geçirilmeli, kullanılan ampirik tedaviler ve profilaktik antibiyotik seçimi tekrar değeriendirilmelidir. Bu çalışmada, üriner enfeksiyon sağaltımında bölgesel antibiyotik direncinin belirlenmesi, antibiyotik seçimine yön verilmesi planlanmıştır. Bu amaçla Çocuk Sağlığı ve Hastalıkları (ÇSH) ve Çocuk Cerrahisi servis ve polikliniklerinden son altı ayda gönderilen 4124 idrar kültürü retrospektif olarak değerlendirilmiş, üreyen mikroorganizmalar ve bunlara karşı antibiyotik direnci araştırılmıştır. Ayrıca sonuçlar beş yıl önceki hastanemiz verileri ile karşılaştırılmıştır. 1996 ve 2001 verileri karşılaştırıldığında saptanan enfeksiyon ajanlarında belirgin bir değişiklik olmadığı, ancak en sık görülen enfeksiyon etkeni olan E.coli' nin görülme sıklığının %62'den %41.5'e gerilediği görülmüştür. Enfeksiyon ajanlarına antibiyotik direnci değerlendirildiğinde, E.coli için karbapenem grubu antibiyotiklerin daha önce saptanan %1, aminoglikozitlerin ve üçüncü kuşak sefalosporinlerin %10 direncinin aynı oranda devam ettiği, ampisilin direncinin %67'den,%75'e, trimetoprim/sulfametoksazol (TMP/SMX) direncini ise %49'dan %61'e çıktığı görülmüştür. Sonuç olarak, üriner sistem enfeksiyonlarında en az beş yılda bir etken ajanların ve antibiyotik direncinin gözden geçirilmesi, böylece ampirik tedavi ve profilakside kullanılacak antibiyotiklerin yeniden seçilmesi morbiditeyi azalttığı gibi, ekonomik kayıpları da önleyecektir.

Resistance to antibiotics of Aegeon region in urinary infection in pediatric patients

Despite several prohibitions, casual and unnecessary prescribing of antibiotics for urinary tract infections (UTI) as well as other types of infections results in resistance to antibiotics and in new infections caused by resistant strains in our country. Hence, resistance to antibiotics* of the geographical area should be investigated, the current antibiotic regimens and the selection of antibiotics for prophylaxis should be re-evaluated.The objective of our study was to determine the local resistance rates to antibiotics and to give a direction for the selection of antibiotics in UTI treatment. We evaluated 4124 urinary culture assays retrospectively, sent from Pediatrics and Pédiatrie Surgery inpatient and outpatient clinics of our hospital during the last six months, and investigated the isolated pathogens and the resistance rates to antibiotics. In addition, the data obtained were compared with that of five years ago. There is no significant difference between the results we obtained and of 1996. However, the frequency rate of the most common UTI pathogen, E. coli, decreased from 62% to 41.5%. With respect to the resistance rates to antibiotics of UTI pathogens, the resistance rates of E. coli for carbapenems, aminoglycosides and 3? generation cephalosporins were 1%, 10% and 10% respectively as before, but the rates for ampicillin increased from 67% to 75% and for TMP-SMX it increased from 49% to 61%. We concluded that the resistance profiles to antibiotics should be reviewed every five years at least, and thus the selection of proper antibiotics would lessen the morbidity as well as the medical expenses.
