Adrenal bezde myelolipom: Olgu sunumu
Myelolipom adrenalde, yağ ve kemik iliği elemanlarını değişen oranlarda bulunduran, nadir, benign bir tümördür. Myelipom sıklıkla asemptomatik ve çoğunlukla ilişkisiz bir nedenle yapılan görüntüleme yöntemleri sırasında veya otopsi sırasında rastlantı ile bulunur. BT'de 10 cm çaplı, nonhomojen yağ dansitesinde kitle ortaya konuldu. Preoperatif l!AB ve operasyon sırasında kitleye insizyonel biopsi uygulandı. llAB tanısı benign, kitleden yapılan insizyonel biopsinin tanısı ise myelolipom idi. Bugüne dek hiçbir kuram, adrenal bezde yağ dokusu ile kemik iliği dokusunun varlığını başarı ile açıklayamamıştır. Malign veya metastatik adrenal bez tümörü olasılığı ekarte edilemiyorsa USG veya BT eşliğinde llAB yapılması yararlı olabilir. Bu lezyonun az görülmesi nedeniyle, 46 yaşındaki bir kadında mevcut myelolipom olgusu sunulmaktadır.
Myelolipoma in the adrenal gland: Case report
Myelolipoma of the adrenal gland is a rare, benign, nonfunctioning tumor consisting of fat and bone marrow elements in varying proportions. This tumor is commonly asymptomatic and usually discovered during various diagnostic-imaging examinations performed for unrelated diseases or it is found incidentally at autopsy. No endocrinologie abnormalities were demonstrated. CT revealed a nonhomogenous mass 10-cm in diameter that consisted of tissue with a fatty density. FNAB and incisional biopsy were performed. The cytologie diagnosis of FNAB was benign, and the histological diagnosis was ad-renal myelolipoma. None of the many theories advanced up to day has succeeded in explaining how these myeloid and adipose tissues occur in the adrenal gland. If a primary malignant or metastatic adrenal tumor cannot be excluded, ultrasound- or computed tomography-guided needle biopsy of the tumor is useful. We report a case of adrenal myelolipoma occurring in a 46-year-old-woman because of the rarity of this entity.
