Ablatio plasenta vakalarında perinatal sonuçlar ve plasenta bulguları

Amaç: Ablasyo plasenta olgularının perinatal sonuçları ve plasental bulguları, ablasyo plasenta saptanmayan grubun sonuçları ile karşılaştırılarak değerlendirildi. Gereç ve Yöntem: Ege Üniversitesi Kadın Hastalıkları ve Doğum Kliniği’nde Nisan 2010-Haziran 2011 tarihleri arasında doğum yapan 1104 hasta içinde saptanan 11 ablatio plasenta vakasına ait fetal sonuçlar ve plasenta bulguları; diğer gebelerle karşılaştırmalı ve retrospektif olarak değerlendirilmiştir. Bulgular: Preeklampsi, tip I diabetes mellitus, önceki gebelikte ablatio plasenta öyküsü ablatio plasenta grubunda fazla bulunmuştur. Ablatio plasenta vakalarında doğum ağırlığı, doğumda gebelik haftası, birinci ve beşinci dakika APGAR skorları düşük saptanırken, yenidoğan ünitesinde yatış ihtiyacı anlamlı olarak yüksek bulunmuştur. Ablatio plasenta grubunda plasenta ağırlığı anlamlı olarak az bulunmuştur. Umbilikal kord uzunluğu, kordda gerçek ya da yalancı düğüm, kordon striktürü, kord insersiyonu, boyna kordon dolanması, iki grup arasında plasenta previa rastlanma oranı ve plasental adezyon defekti varlığı açılarından anlamlı fark bulunmamıştır. Her iki grup hiperkoiling ve hipokoiling açısından da değerlendirilmiş, aralarında anlamlı fark bulunmamıştır. Sonuç: İzole yapısal plasental patolojiler açısından, ablasyo plasenta saptanan ve saptanmayan hastalar arasında farklılık bulunmamaktadır.

Perinatal outcomes and placental findings in ablatio placentae

Aim: The purpose of this study is to compare the perinatal outcomes and placental findings of patients with and without ablatio placentae. Materials and Methods: 1104 patients who gave birth in between April 2010-June 2011 at the Ege University Department of Obstetrics and Gynecology were evaluated retrospectively in this study and 11 ablatio placentae cases were compared with the other patients. Results: Preeclampsia, diabetes mellitus type I, history of ablatio placentae in the previous pregnancy are significantly higher in the ablatio placenta group. In ablatio group, birth weight, time of delivery and the APGAR scores at the first and the fifth minute were significantly lower, where the need for the neonatal intensive care unit was significantly higher. Placental weight is significantly lower in the ablatio group. The length of umbilical cord, knot, stricture, cord entanglement and the incidence of placenta previa and adhesion defects do not differ in the two groups. There is no significant difference for umbilical cord hyper/hypocoiling between the two groups. Conclusion: The incidence of isolated placental pathologies does not differ in the patients with ablatio placentae or the other groups.
