Yoğun bakım hastalarında uygulanan santral venöz kateterlere bağlı kolonizasyon değerlendirme
Yoğun bakım ünitelerinde izlenen hastalarda görülen enfeksiyonların bir kısmından santral venöz kateterler (SVK) sorumludur. Çalışmamızda yoğun bakım ünitemizde uyguladığımız SVK'lerin uçlarında oluşan mikroorganizma kolonizasyon oranlarını saptamak ve enfeksiyon etkenlerini analiz etmeyi amaçladık. Yoğun bakım ünitemizde bir yılda uyguladığımız vena basilica (n=25), vena subclavia (n=5), vena jugularis interna (n=4), vena femoralis (n=6) olmak üzere toplam 40 kalelerin sonuçlarını değerlendirdik. Kateterlerin ortalama kalış süresi 8.4±5. 1 gün (2-23 gün) idi. Kateterlerin giriş yerleri hergün periton-iyot (Betadine®) ile pansuman edilip, şeffaf pansuman (Tegaderm®) ile kapatıldı. Değerlendirmeye alınan 40 kateter ucundan 6 tanesinde (%15) enfeksiyon etkeni saptandı. (Acinetobacter spp. (n=2). koagülaz (-) stafilococcus (n=2), maya benzeri mantar (n=1), oxaciline dirençli stafilococcus aureus koagülaz (+) (n=1). 34 kateter ucunda ise üreme saptamadık (%85). Bu çalışmada, uygulama ve günlük bakımın steril koşullarda yapılması halinde, SVK uçlarında oluşan kolonizisyona bağlı enfeksiyonların azalabileceği sonucuna vardık.
Central venous catheters (CVC) are responsible for a part of the infections encountered in intensive care units (ICU). A study, has been conducted to determine the colonisation rates of microorganisms at tip of CVC and the agents of infection. Forty specimens, were sent to the microbiology laboratory. The insertion sites of CVC were the following veins: basilic (n=25), subclavian (n=5), internal Jugular (n=4) and femoral (n=6). The mean duration of catheterization was 8.4±5.1 days (ranse:2-23 days). Cannulation sites were cleaned with povidone-iodine (Betadine®) every day and sterile transparent dressing (Tegaderm®) were applied. In 6 of 40 catheter tips a potential pathojen was isolated (15%). The agents were: Acinetobacter spp (n=2), coagulase (-) staphylococci (n=2), "yeast-like microorganism" (n=1), oxacilline resistant staphylococcus aureus coagulase (+) (n=1). There was no bacterial growth in the other 34 catheters (85%). We conclude that the ratio of infections due to colonization at the tip of CVC may be decreased by daily care of catheters performed under sterile conditions.
