Sineözofagografiyle tanı konulan gastro özofajial reflülü tekrarlayan hışıltılı infant

Hışıltı (wheezing), bronş duvarının kısmi daralması sonucu solunum esnasında titreşimiyle ortaya çıkan müzikal sestir. Hışıltı oldukça yüksek olan prevalansı nedeniyle çocuk yaş grubunun en sık görülen semptomlarından biridir. Çocukluk çağında hışıltıya neden olan çeşitli hastalıklar vardır. Doğumundan beri dört kez hışıltılı nefes alma yakınmaları ile çeşitli hastanelere yatarak tedavi gören 4.5 aylık erkek hasta beşinci kez aynı yakınmalarla hastanemize başvurdu. Tekrarlayan hışıltı yapan nedenlere yönelik yapılan tetkiklerden sineözofagografi esnasında belirgin gastro-özofajial reflü(GÖR) saptandı. GÖR'ye yönelik tedavi sonrası hışıltı yakınmaları kayboldu. Özellikle bebeklerde ve küçük çocuklarda hışıltının ayırıcı tanısında gastro-özofajial reflü mutlaka düşünülmelidir. Ayrıca GÖR tanısında deneyimli ellerde sineözofagografininde yararlı olabileceğini belirtmek için olguyu sunduk.

Wheezing is a musical voice due to vibration of the narrowed bronchial wall during the breating. It is one of the most common symptoms of the childhood because of its high prevalance. In the childhood there are different kind of diseases due to wheezing. The male patient who has been treated for wheezing four times in different hospitals since birth date admitted to our hospital with same complaint. Gastroesophageal reflux (GER was determined by cineesophagogram which is a diagnostic tool to use for investigation of recurrence of -wheezing. Wheezing has improved after GER treatment. Especially in infants and children GER should be considered differential diagnosis of wheezing. We presented the case to determine that cineesophagogram is a useful test in the skilled hands to diagnose GER.
