Endometrial karsinomlarda bcl-2, nm23 ve E-Cadherin ekspresyonlarının yaş, histolojik tür, derece, evre ve myometrial invazyon derinliği ile ilişkisi
Amaç: Endometrium kanseri kadın genital sisteminin en sık görülen invaziv tümörüdür. Tümörün histolojik türü, derecesi, evresi, myometrial invazyon derinliği ve lenf düğümü tutulumu endometrial kanserde prognostik önemi bilinen primer faktörlerdir. Bu histolojik parametrelerin yanısıra; bazı onkogen ve tümör supressör genlerin de karsinom gelişimi ve prognozun belirlenmesinde rolü olduğu düşünülmektedir. Bu çalışmada 71 endometrial karsinom olgusunda-bcl-2, E-Cadherin ve nm 23 ekspresyonu ile diğer histopatolojik parametreler arasındaki ilişkinin araştırılması amaçlanmıştır. Gereç ve yöntem: Bu çalışmada endometrial karsinom nedeniyle radikal ameliyat uygulanmış 71 olgu yer almaktadır. Bu olgulara ait histopatolojik parametreler yeniden gözden geçirilmiş ve tümörden hazırlanan kesitlere streptavidin-biotin yöntemi ile bcl-2, nm 23 ve E-Cadherin ile immunboyama yapılmıştır. Sonuçlar istatistiksel olarak; Spearman, Ki-Kare ve One-Way Anova korelasyon testleri ile değerlendirilmiştir. Bulgular: E-Cadherin ve bcl-2; diğer tiplere göre daha iyi prognoz gösteren endometrioid adenokarsinomlarda, erken evre ve iyi differansiye tümörlerde yüksek derecelerde eksprese edilmiştir. Ayrıca E-Gadherin ekspresyonu, myometrial kalınlığın 1/2'sinden daha az invazyon gösteren olgularda daha fazla saptanmıştır. Ancak nm 23 ekspresyonu ile bu histopatolojik parametreler arasında istatistiksel olarak anlamlı bir ilişki izlenmemiştir. Sonuç: Endometrial karsinomlarda E-Cadherin ve bcl-2 ekspresyonundaki artışın; daha iyi davranışlı histolojik tür, düşük derece ve erken evre gibi iyi prognostik faktörler ile birliktelik gösterdiğini düşünüyoruz.: Ayrıca E-Cadherin ekspresyonundaki artış myometrial invazyon derinliğinin azalması ile ilişkili bulunmuştur.
The relationship of the expression of bcl-2, nm23 and E-Cadherin with histologiacal type, grade, stage, age and depth of myometrial invasion in endometrial carcinomas
Objective: Endometrial carcinoma is the-most common gynaecologic cancer in the women. Histologie type, grade, stage, depth of myometrial invasion and nodal status are established primary factors in assessing the prognosis of endometrial cancer. In recent years, changes in the expression of protooneogenes, tumor supressor genes and adhesion molecules in carcinomas have investigated by researchers. We studied 71 endometrial cancer if there is any relationship between bcl-2, E-cadherin, nm 23 expression and histopathologic parameters. Material and methods: The archival tissues of radical materials of 71 cases with endometrial carcinoma were studied. The tumor tissues obtained from archival paraffin blocks were originally fixed in formalin. The blocks were sectioned and mounted on poly-L-lysin coated slides. The avdin-bio-tin-peroxidase method was per-formed using the primary mono- clonal antibody against bcl-2, nm 23 and E-Cadherin. Results: E-cadherin and bcl-2 expressions were higher in endometrioid type adeho-carcinomas, early stages and low grade tumors than the others. In addition; E-cadherin expression was most strongly correlated with depth of myometrial invasion. There was no statistical relationship between nm 23 expression and any of the histopathologic parameters. Conclusion: We thought, bcl-2 and E-cadherin expression in endometrial carcinoma might be associated with good prognosis. We found negative correlation between E-cadherin expression and depth of myometrial invasion. These results suggest that decreased expression of E-Cadherin facilitates invasion of endometrial carcinoma.
