Az invaziv pektus ekskavatum onarımı (Nuss yöntemi)

Amaç: Pektus ekskavatumun (PE) klasik ameliyatı olan "Ravitch" yöntemine alternatif olarak minimal invaziv bir ameliyat yöntemi geliştirilmiştir. "Nuss" yöntemi ile pektus ekskavatum kıkırdak rezeksiyonu ve osteotomilere gerek kalmadan dü¬zeltilebilmektedir. Bu yöntemin 15 yıllık sonuçları iyi kozmetik sonuçlar alındığını göstermektedir. Gereç ve yöntem: Haziran 2002-Kasim 2005 tarihleri arasında "Nuss" yöntemi ile pektus ekskavatum onarımı uygulanan, yaşları 5 ile 18 yaş arası (ortanca 9 yaş) değişen 6 simetrik pektus ekskavatumlu hastanın (1 kız, 5 erkek) dosya kayıtları geriye dönük olarak incelendi. Bulgular: Onbeş ve 18 yaşındaki iki hasta dışındaki dört hastaya tek destek çubuğu yerleştirilirken bu iki hastaya iki destek çubuk yerleştirildi. Tüm hastaların ameliyat sırasında ve sonrası takiplerinde bir komplikasyonla karşılaşılmadı. Üç hastanın destek çubukları ikinci yılları dolduktan sonra sorunsuz bir şekilde çıkarıldı. Diğer üç hastanın destek çubuklarının onarım sonrası ikinci yılları dolmadığı için henüz çıkartılmamışlardır. Destek çubuğu çıkarılan üç hastanın da göğüs kafesi onarım sonrası yeni şeklini korumaktadır. Sonuç: "Nuss" yöntemi, az kan kaybına neden olması, az skar bırakarak iyileşmesi ve ameliyat süresinin kısa olması nedeni ile gerek hastalar ve ebeveynleri tarafından gerekse doktorlar tarafından tercih edilmektedir. Kısa dönem sonuçlarının yüz güldürücü olması bu yöntemin tercih edilmesini destekler niteliktedir.

Minimally invazive pectus excavatum repair (Nuss procedure)

Objective: Minimally invasive pectus excavatum repair technique was developed as an alternative to standart "Ravitch" technique. With "Nuss" procedure, pectus excavatum can be repaired without cartilage resections and osteotomies. The 15 years experience with "Nuss" procedure shows that, the cosmetic results are satisfactory. Material and method: The hospital records of 6 patients (1 girl and 5 boys) aging 5years to 18 years (median 9 years) who underwent "Nuss" procedure during June 2002-November 2005 were evaluated retrospectively. Results: Apart from the 15 and 18 years old patients who had two support bars placed into their thorasic cavity, one support bar was placed to the remaining four children. Peroperative and postoperative period of the patients were uneventful. The support bars of the first three patients had been removed without any problems after two years. The remaining three patients are still carrying their bars. Chest walls of the three children have been maintaining their new shape after the bar removal. Conclusion: Nuss repair has been appealing to the patients, parents and surgeons with lesser blood loss, small scar occurrence and the shorter operation time. Because of its excellent short term results, it can be the preferred operation method.
