Kronik Viral Hepatit Kaynaklı Karaciğer Sirozlu Bir Hastada Bruselloz Olgusu

Bruselloz ülkemizde endemik olarak görülen bir enfeksi- yon hastalığıdır. Klinik belirti ve bulgular geniş bir spekt- ruma yayılmakta; kas-iskelet sistemi, gastrointestinal sistem, kardiyovasküler sistem ve genitoüriner sistem gibi birçok sistem etkilenebilmektedir. Brusellozda kara- ciğer ve dalak gibi retiküloendotelyal sistem organların- daki tutuluma bağlı olarak transaminaz yüksekliği sıklıkla görülmektedir. Bizim olgumuzda transaminaz yüksekliği, bisitopeni ve ateş ile başvuran hastada transaminaz yük- sekliğine yol açabilecek viral nedenlerin atlanmaması için yapılan tetkiklerde hastada kronik hepatit B ve delta he- patiti sonucu gelişen karaciğer sirozu saptanmış olup bru- selloz ve hepatit Bnin sık görüldüğü bölgemizde bu olası durumların göz ardı edilmemesi gerektiği vurgulanmıştır

Chronic Viral Hepatitis-Induced Liver Cirrhosis in a Patient with Brucellosis

Brucellosis is an endemic infectious disease in our coun- try. Clinical signs and symptoms are spread over a wide spectrum; musculoskeletal, gastrointestinal system, car- diovascular system and the genitourinary system can be affected. Elevation of transaminases is a common finding in brucellosis due to involvement of reticuloendothelial system including the liver and spleen. In the patient pre- sented here, hepatic cirrhosis was found due to chronic hepatitis B and delta hepatitis infection in the investiga- tions made for not to overlook viral causes that might cause transaminase elevation and it was emphasized that these possible conditions shouldnt be overlooked in our region where brucellosis and hepatitis B are common.
