İnfertil Bir Erkekte “De La Chapelle” Sendromu: Olgu Sunumu
46 XX erkek sendromu çok nadir görülen bir hastalık olup, ilk olarak 1964 yılında De La Chapelle ve arkadaşları tarafından tanımlanmıştır1. Hastalar normal genital yapıdan ambiguus genitalya kadar değişebilen bir genital görünüme sahip olabilirler2. Y kromozomundaki cinsiyet belirleyen SRY genini içeren bir parçanın X kromozomuna translokasyonuyla geliştiği düşünülen bir hastalıktır. Genellikle erkek fenotipinde gelişme olur ve Y kromozom eksikliğine bağlı testiküler yetmezlik tablosu gelişir2. Otuz dört yaşında 10 aydır evli bir erkek hasta infertilite nedeniyle polikliniğimize başvurdu. Fizik muayenesinde bilateral testislerin atrofik olduğu izlendi. Hastada hipergonadotropik hipogonadizm ve azoospermi saptandı. Hastanın karyotip analizi 46,XX olarak saptandı ve X kromozomunda SRY sinyali mevcuttu. Moleküler analizde hastanın Y kromozomunun p kolunun SRY ve ZFY bölgelerini içerdiği saptandı. Ayrıca AZFa, AZFb, AZFc delesyonu bulunduğu görüldü. Bu hastalarda spermatogenez olmadığından testis biyopsisi ve spermiogram uygun değildir8. Hastalıkta başlıca tedavi testosteron replasmanıdır. Ayrıca profesyonel bir genetik danışmanlık için hastanın yönlendirilmesi uygundur. De La Chapelle sendromu, infertilite ile polikliniğe başvuran bilateral testis atrofisinin eşlik ettiği primer hipogonadizmli hastaların ayırıcı tanısında akılda tutulması gereken bir hastalıktır.
“De La Chapelle” Syndrome In An Infertile Male: A Case Report
46 XX male syndrome is a very rare disease, first described by De La Chapelle et. al in 19641. Patients may have a genital appearance that varies from normal genital to ambiguus genitalia. It is thought that a fragment containing the sex-determining SRY gene in Y chromosome is believed to develop by translocation into the X chromosome. Usually the male phenotype and a testosterone deficiency due to Y chromosome deficiency develops2. A 34-year-old male patient married for ten months applied to our clinic for infertility. Bilateral testes were atrophic on physical examination. Hypergonadotropic hypogonadism and azoospermia were detected in the patient. The patient's karyotype analysis was 46, XX and there was an SRY signal on the X chromosome. In molecular analysis, the patient's Y chromosome was found to contain SRY and ZFY regions of the p-arm. It was also observed that there were AZFa, AZFb, AZFc deletions. Because these patients do not have spermatogenesis, testis biopsy and spermogram are not appropriate8. The main treatment in the disease is testosterone replacement. It is also advisable that the patient be directed for professional genetic counseling. De La Chapelle syndrome is a rare disease that should be kept in mind in the differential diagnosis of patients with primary hypogonadism who are admitted to polyclinics with infertility.
