Çok ileri yaşlı hastalarda (75 yaş ve üzeri) perkütan nefrolitotomi

Amaç: Perkütan nefrolitotomi (PNL),tüm dünyada böbrek taşı cerrahi tedavisinde güvenle kullanılan minimal invaziv bir yöntemdir. İlk PNL ameliyatı Mart 1998 tarihinde yapılan kliniğimizde yetmiş beş yaş üzeri ilk PNL ameliyatı ise Haziran 2000 tarihinde bilateral olarak yapıldı. Çalışmamızda, 75 yaş ve üstü hastalara yapılan PNL operasyonları incelendi. Yöntemler: PNL işlemleri genel anestezi altında yapıldı. Hastalara, supin pozisyonda sistoskopi yapıldıktan sonra üreter orifisinden üreter kateteri C kollu floroskopi eşliğinde takılarak üretral foley katetere tespit edildi. Sonrasında hastalar prone pozisyona alınarak floroskopi eşliğinde Chiba iğnesi ile böbreğe girildi. Amplatz renal dilatatör setiyle 30F’e kadar dilatasyon sağlandı. İşlem sonrası renal kılıf içerisinden nefrostomi kateteri yerleştirildi. PNL sonrası 4 mm’den küçük taşlar, klinik önemsiz taş rezidüleri kabul edildi. Bulgular: Mart 1998 – Aralık 2014 tarihleri arasında, 16 yılda toplam 3003 perkütan nefrolitotomi işlemi yapıldı. Bu hastalardan 27 tanesi 75 yaş ve üzeriydi. Bu hastalara, biri bilateral olmak üzere 28 PNL işlemi yapıldı. Hastaların yaş ortalaması 79,37 (75-88) yıl olarak hesaplandı. 20 (%74) hastada tam taşsızlık sağlandı. 2 hastada tubeless PNL yapıldı. Viseral organ yaralanması, sepsis ya da ölüm görülmedi. Sonuç: Minimal invaziv bir yöntem olan PNL, yüksek güvenlik, düşük komplikasyon oranları, daha kısa hastanede kalış süresi, yüksek hasta memnuniyeti sağlaması, daha yüksek taşsızlık oranları ile açık böbrek taşı cerrahisine göre avantajlı bir hale gelmiştir. Üriner sistem taş hastalığının cerrahi tedavisinde PNL güvenli ve etkili bir yöntemdir.

Percutaneous nephrolithotomy in very elderly patients (Age 75 and Over)

Objective: Percutaneous nephrolithotomy (PNL) is a minimally invasive method used safely in surgical treatment of kidney stones in the world. The first PNL surgery performed in our clinic in March 1998 and first PNL surgery was performed in June 2000 over the age of seventy which was bilaterally side. In our study, PNL operations performed for 75 years and older patients were examined. Methods: PNL operations were performed under the general anesthesia. After cystoscopy in the supine position, ureteral catheter was inserted accompanied by C-arm fluoroscopy. Afterwards, patients were taken prone position then we entered into the kidney with a metal needle with fluoroscopic on. We dilated the entrance with Amplatzer renal dilatators set until the 30 F. After the procedure, the nephrostomy catheter was placed in the renal sheath. 4 mm residual fragments after the PNL operation were accepted as clinically insignificant stone residues (CIRF). Results: 3003 PNL procedures were performed between dates March 1998 - December 2014 in total 16 years. 28 patients of total were 75 years and over. 29 PNL surgeries performed to these patients, including one bilateral PNL procedure. Mean age was 79.36 years (75-88). Twenty (74%) of the patients were concluded stone free. 2 patients had tubeless PNL surgery. There was no another complication. Conclusion: PNL is a minimally invasive method has became advantageous according to open surgery because of higher safety, lower complication rates, shorter hospitalization stay, more patient comfort and higher stone free rates. PNL is a safe and effective method in the surgical treatment of urinary tract stone diseases.
