Oligohidramniyosun neonatal etkileri
Amaç: Prenatal dönemde yapılan ultrasonografide saptanan oligohidramniyos ile perinatal morbidite ve renal anomali ilişkisini araştırmak. Gereç ve Yöntem: Mart 2001-Mart 2002 tarihleri arasında intrapartum yapılan ultrasonografilerde amniyotik sıvı indeksi ? 5 cm olan vakalar oligohidramniyos, amniyos sıvısı hiç olmayanlar ise anhidramniyos olarak değerlendirildi. Bu bebeklerin tümüne yaşamın ilk 3 günü içinde renal ultrasonografi yapıldı. Amniyos sıvısında azalma olan yenidoğanlar (grup 1) ile amniyos sıvısı normal olan yenidoğanlar (grup 2) doğum şekli, prematürelik, intrauterin büyüme geriliği (IUBG), asfiksi ve mekonyum aspirasyonu sendromu (MAS) sıklığı açısından Student-t testi ile karşılaştırıldı. Ayrıca Grup 1'deki yenidoğanlarda renal anomali varlığı araştırıldı. Bulgular: Toplam 19,546 canlı doğumun 171'inde (% 0.84) oligohidramniyos, 28'inde (% 0.14) anhidramniyos, 5'inde (% 0.02) polihidramniyos saptandı. Toplam 199 (%1) vakanın 91'i kız, 108'i erkek, doğum tartısı ortalaması 2874±47 g'dı. Grup 1'deki yenidoğanların 86'sı (% 43.2) normal spontan doğum (NSD) ile, 103'ü (% 56.8) sezaryen ile doğarken, grup 2'deki vakaların 13,443'ü (% 69.5) NSD, 5,889'u (% 30.5) ise sezaryen ile doğmuştu. Vakaların 34 (% 17)'ü prematüre olup, 41 (% 21.1) bebekte IUBG mevcuttu. Aynı dönemde grup 1'deki prematürelik oranı % 2.4, IUBG oranı ise % 9.3 saptandı. Asfiksi, grup 1'deki vakaların 11'inde (% 5.5), grup 2'deki yenidoğanların ise 19'unda (% 0.1) saptandı. MAS açısından karşılaştırılma yapıldığında grup 1'deki yenidoğanların 1'ine (% 0.4), grup 2'deki yenidoğanların ise 19'una (% 0.1) MAS tanısı kondu. Bütün parametreler için aradaki fark istatistiksel olarak anlamlı bulundu (p< 0.0001). Postnatal dönemde yapılan ultrasonografilerde toplam 10 (% 5) vakada renal patoloji saptandı. Sonuç: Amniyos sıvısında azalma ile sezaryen doğum, prematürelik, IUBG, asfiksi ve MAS riskinin arttığı görülmektedir. Ayrıca oligohidramniyosu olan vakalarda prenatal ve postnatal dönemde renal anomalilerin aranması unutulmamalıdır.
Neonatal effects of oligohydramniosis
Aim: To investigate the relationship between amniotic fluid decrease detected by intrapartum ultrasonographic examination and perinatal morbidity and renal anomalies, in the newborn baby. Material and Methods: The study was conducted between March, 2001 and March, 2002. The cases with an amniotic fluid index less than 5 cm were evaluated as oligohydramniosis, and the cases with no amniotic fluid as anhydramniosis. All newborn babies were examined by renal ultrasound within the first 3 days of their life. The newborns with decreased amniotic fluid (group 1) and normal amniotic fluid (group 2) were compared in terms of mode of delivery, prematurity, intra uterine growth retordation (IUGR), frequency of asphyxia and meconium aspiration syndrome. Renal anomalies were investigated in group 1. Results: Among a total of 19,546 live births, 171 (0.84 %) had oligohydramniosis, 28 (0.14 %) had anhydramniosis and 5 (0.02 %) had polyhydramniosis. When the delivery mode of 199 (1 %) cases with low amniotic fluid studied, 43.2 % of group 1 newborns were born by vaginal delivery and 56.8 % by cesarean section, 69.5 % of group 2 cases by vaginal delivery and 30.5 % by cesarean section. 17 % of group 2 cases were premature babies and 21.1 % of the babies had IUGR. During the same period, in group 1, the prematurity and IUGR rates were 2.4 % and 9.3 % respectively. Asphyxia was found in 5.5 % of group 1 and in 0.1 % of group 2. As for MAS, the newborns in group 1 were diagnosed as MAS with a ratio of 0.4 % and the ones in group 2 with a ratio of 0.1 %. The difference was statistically significant for all parameters (p<0.0001). A renal pathology was found in 10 cases. Conclusion: The risks for cesarean section, prematurity, IUGR, asphyxia and MAS increased in cases with oligohydramniosis. Renal anomalies should also be investigated in cases with oligohydramniosis in prenatal and postnatal period.
