Gebelikte madde suiistimalinin yenidoğana etkisi

Gebelikte madde suistimali, yenidoğan bebekte ciddi hastalıkların gelişimine zemin hazırlar. Yenidoğanın yoksunluk sendromu olarak adlandırılan klinik tablo, madde kullanan kadınların bebeklerinde doğum sonrası madde geçişinin ani olarak kesilmesiyle ortaya çıkmaktadır. Başlıca yoksunluk bulguları; hiperirritabilite, solunum güçlüğü, tremor, ateş, yüksek sesle ağlama, dehidratasyon ve elektrolit dengesizliğidir. Ayrıca, bağımlı anne bebekleri genellikle düşük doğum tartısı ve düşük Apgar skorlarına sahiptir. Makalemizde opiat bağımlısı annelerden doğan iki yenidoğan vakası sunuldu. İlk bebekte yaşamının on altıncı saatinde yenidoğan yoksunluk bulguları saptandı ve fenobarbital tedavisi ile başarıyla tedavi edildi. Preterm olan ikinci bebekte, şiddetli büyüme geriliği ve ağır solunum yetersizliği vardı. Pulmoner ve intraserebral kanama ile kaybedildi.

Neonatal effects of drug abuse during pregnancy

Use of addicting drugs among women during pregnancy exposes newborns to potentielly serious disorders. A group of symptoms referred to as neonatal withdrawal symptoms (NWS) may occur in infants born to mothers addicted to drugs because at birth the infants are suddenly cut off from the supply. NWS includes hyperirritability, respiratory distress, tremors, fever, high-pitched cry, dehydration and electrolyte imbalance. In addition to NWS, infants born to addicted women are usually low birth weight and have low Apgar scores. We report two newborns born to opioid addicted mothers. NWS were seen in the first baby at 16th hours of life, treated succesfully with phenobarbital. The second baby was born preterm with severe growth retardation, presented with severe respiratory insufficiency and died of pulmonary and intracerebral hemorrhage.
