Bez dermatiti nedeniyle kullanılan lokal steroide bağlı gelişen iatrojenik cushing sendromlu beş vaka

Bez dermatiti, sütçocuklarında en yaygın görülen hastalıklarından biri olup, en sık 9-12. aylarda rastlanır. Bez dermatitinin hafif formları kendiliğinden düzebilirken daha ciddi formları tıbbi tedaviye gereksinim duyar. Diğer topikal ajanlara yanıt alınamayan orta ve ciddi bez dermatitlerinde kısa süreli, düşük potensli kortikosteroidler yararlı olabilir. Kullanılan kortikosteroid potent olduğunda veya uzun süreli kullanıldığında iatrojenik Cushing sendromuna neden olabilir. Bu çalışmada bez dermatiti tedavisi sırasında, topikal potent kortikosteroid olan klobetazol 17 propionat (Dermovate % 0.05 krem) kullanımı sonucu, iatrojenik Cushing sendromu gelişen beş kız vaka sunulmuştur.

Five cases of Iatrogenic cushing’s syndrome due to the use of a topical steroid in diaper dermatitis

Diaper dermatitis is one of the most common disorders in infants. The incidence of diaper dermatitis is highest in those between 9 and 12 months of age. While mild forms heal spontaneously, more severe forms may require medical attention. A short duration of mild corticosteroid application may be useful in moderate to severe cases of diaper dermatitis which have not responded to other topical agents. Iatrogenic Cushing’s syndrome may be due to extended use of steroid or the potency of topical corticosteroids. We report five cases of Cushing’s syndrome due to the use of a topical potent corticosteroid (clobetazole 17 propionate 0.05 % cream) in diaper dermatitis.
