STAPHYLOCOCCUS AUREUS SUŞLARINDA METİSİLİNE DİRENÇ ORANINDAKİ DEĞİŞİM: METİSİLİNE DİRENÇ AZALIYOR MU?

Staphylococcus aureus, tüm dünyada morbidite ve mortalitesi yüksek enfeksiyonlara neden olabilen önemli bir bakteridir. S.aureus suşlarında metisiline karşı direncin ortaya çıkmasından sonra, metisiline dirençli S.aureus (MRSA) klonları dünyada ciddi nozokomiyal enfeksiyonlara neden olmuştur. Doksanlı yıllarda ise toplumda, cilt ve yumuşak doku enfeksiyonlarına neden olan farklı S.aureus klonları ortaya çıkmıştır. Günümüzde MRSA, hastanelerde olduğu kadar toplumda da çeşitli enfeksiyonlara yol açan önemli bir enfeksiyon etkeni haline gelmiştir. Çalışmamızın amacı, Ağustos 2007 - Temmuz 2008 tarihleri arasında izole edilmiş S.aureus suşları ile 2016 yılı içerisinde izole edilmiş suşların metisiline direnç oranlarını karşılaştırmaktır. Çalışmamıza dahil edilen 376 S.aureus suşunun 100’ü Ağustos 2007 - Temmuz 2008 tarihleri arasında, 276’sı 1 Ocak 2016 - 31 Aralık 2016 tarihleri arasında Adnan Menderes Üniversitesi Tıp Fakültesi Tıbbi Mikrobiyoloji Anabilim Dalı Laboratuvarı’na gönderilen klinik örneklerden izole edilmiştir. S.aureus suşlarının tanımlanmasında tam otomatik bakteri tanımlama sistemi Phoenix® (BD Diagnostic Systems, ABD), DNaz, mannitol tuz agarda üreme ve koagülaz testleri kullanılmıştır. Antimikrobiyal duyarlılık, Phoenix® (BD Diagnostic Systems, ABD) tam otomatik duyarlılık sistemi ve disk difüzyon yöntemi ile belirlenmiştir. Duyarlılık sonuçları Clinical Laboratory Standards Institute (CLSI) ve The European Committee on Antimicrobial Susceptibility Testing (EUCAST) kılavuzlarına göre yorumlanmıştır. Çalışmamızda, Ağustos 2007 - Temmuz 2008 tarihleri arasında elde edilen S.aureus suşlarının % 34’ü metisiline dirençli bulunmuştur. 2016 yılında ise S.aureus suşlarında metisiline direnç % 15.3 olarak bulunmuştur. The Change of the Methicilline Resistance Ratio in Staphylococcus aureus Strains: Is There a Decrease in Methicillin Resistance?

Staphylococcus aureus is a major healthcare problem that can cause infections with high morbidity and mortality rates worldwide. Since, discovery of resistance to methicillin in S.aureus strains, the methicillin resistant S.aureus (MRSA) clones spread to the world and caused serious nosocomial infections. In 90s, different clones of S.aureus that often cause skin and soft tisue infections arose in community. Currently MRSA is an important infectious agent that causes various infections in community as well as healthcare settings. The aim of this study was to compare the antimicrobial resistance profiles of S.aureus strains that were isolated in 2016 and between August 2007 August - July 2008. A total of 376 S.aureus strains were isolated from clinical specimens in Adnan Menderes University Medical Faculty Medical Microbiology Laboratory. A hundred strains were isolated between August 2007 - July 2008 and, 276 strains were isolated in 2016. Fully automated bacterial identification system, Phoenix® (BD Diagnostic Systems, USA), DNase, growth on mannitol salt agar and coagulase tests were used for identification of S.aureus strains. Antimicrobial susceptibilities were determined by the Phoenix® (BD Diagnostic Systems, USA) full automated sensitivity system and disk diffusion method. Susceptibility results were interpreted according to the guidelines of the Clinical Laboratory Standards Institute (CLSI) and The European Committee on Antimicrobial Susceptibility Testing (EUCAST).
