METİSİLİNE DİRENÇLİ STAPHYLOCOCCUS AUREUS SUŞLARININ ANTİBİYOTİKLERE DUYARLILIĞI, HASTALARIN KLİNİK ÖZELLİKLERİ VE MORTALİTEYİ ETKİLEYEN FAKTÖRLER
Staphylococcus aureus, toplum ve hastane kaynaklı infeksiyonlara yol açabilen önemli bir patojendir. Hekimler, özellikle metisiline dirençli S.aureus (MRSA) suşlarının neden olduğu infeksiyonların tedavisinde sorunlar yaşamaktadır. Bu çalışmada klinik örneklerden izole edilen MRSA suşlarının, çeşitli antibiyotiklere duyarlılıkları, hastaların klinik özellikleri ve prognozu etkileyen faktörlerin retrospektif olarak irdelenmesi amaçlanmıştır. Ocak 2010 - Haziran 2013 tarihleri arasında iki eğitim ve araştırma hastanesinde MRSA izole edilen 51 hasta çalışmaya dahil edil- miştir. Hastalara ait veriler hasta dosyalarından ve elektronik bilgi sisteminden elde edilmiştir. MRSA suşlarının vankomisin, teikoplanin, linezolid ve daptomisin minimum inhibitör konsantrasyon (MİK) değerleri E-test yöntemiyle belirlenmiş ve duyarlılık durumları değerlendi- rilmiştir. Çalışmaya alınan 51 hastanın % 62.7sinin erkek olduğu ve % 37.3ünün 65 yaş ve üzerinde olduğu görülmüştür. Eşlik eden hastalıklar incelenmiş; % 33.3ünde hipertansiyon, % 27.5inde diabetes mellitus, % 21.6sı kronik böbrek yetmezliği, % 17.6sında serebro- vasküler hastalık, % 11.8inde malignite saptanmıştır. Hastaların % 51i operasyon geçirmiştir. Hastaların % 92.2si yatan hasta olup orta- lama yatış süresi 42.8 gündür. Yatış tarihinden MRSA izole edilmesine kadar geçen süre ortalama 18 gündür. Ayaktan izlenen hastaların tümü son üç ay içinde ortopedik operasyon geçirmiştir. İzole edilen suşların tümü (% 100) hastane infeksiyonu etkeni olarak kabul edilmiştir. Suşların 18 (% 35.3)i yara, 13 (% 25.5)ü kan, 11 (% 21.5)i trakeal aspirat, biri (% 2) kateter, ikisi (% 3.9) balgam, ikisi (% 3.9) ameliyat materyali, ikisi (% 3.9) idrar, biri (% 2) periton sıvısı, biri (% 2) plevral sıvıdan izole edilmiştir. Tüm MRSA suşları vankomisin, teikoplanin, linezolid ve daptomisine duyarlıdır. Antibiyotiklerin MİK50/MİK90 değerleri vankomisin için 0.75/1.5 μg/ml, teikoplanin için 2/4 μg/ml, linezolid için 0.5/1 μg/ml ve daptomisin için 0.125/0.5 μg/mldir.Yedi hastada (%13.7) üreme kolonizasyon kabul edildiği için tedavi verilmemiştir. Beş (% 9.8) hasta izolasyon öncesinde ölüm olduğundan uygun tedavi verilememiştir. Otuzdokuz hasta tedavi edilmiştir ve tedavi yanıt oranı % 61.5tir. Tedavi verilen 20 hasta (% 51.3) ölmüştür. Mortaliteyi artıran faktörler ≥65 yaş olma, mekanik ventilatör kullanımı ve ≥72 saat süreyle gecikmiş antibiyoterapi olarak belirlenmiştir. Sonuç olarak tüm suşlar vankomisine, teikoplanine, linezolide ve daptomisine duyarlı bulunmuş ancak, linezolid ve daptomisinin MİK değerleri daha düşük olduğu saptanmıştır. MRSA infeksiyonlarında vankomisin ve teikoplaninin hala tedavi seçenekleri arasında oldu- ğu görülmüştür. Yoğun bakımda izlenen, mekanik ventilatör gereken, ≥65 yaş hastalarda MRSA infeksiyonlarının daha ağır seyrettiği göz- lenmiştir. Bu hastalarda uygun tedavinin zamanında başlanması mortalite açısından önemlidir.
Antibiotic Susceptibility of Methicillin Resistant Staphylococcus aureus Strains, Clinical Features of Patients and Factors Affecting Mortality
Staphylococcus aureus, is an important pathogen that can cause community and hospital acquired infections. Physicians face problems in treatment of methicillin resistant Staphylococcus aureus (MRSA) infections. In this study it is aimed to evaluate susceptibility of MRSA strains which was isolated clinical specimens to several antibiotics, clinical features of patients and factors effecting prognosis retrospectively. Fiftyone patients from whose specimens MRSA strains were isolated between January 2010 and June 2013 in two training and rese- arch hospitals were included the study. Data of patients were obtained from the patients files and electronic information system. Minimum inhibitory concentration (MIC) values of vancomycin, teicoplanin, linezolid and daptomycin were determined by E-test and susceptibilities of the strains were evaluated. Of 51 patients, 62.7 % were male. 37.3 % of patients were ≥65 years of age. In terms of underlying disease 33.3 % of the patients had hypertension, 27.5 % had diabetes mellitus, 21.6 % had chronic renal failure, 17.6 % had cerebrovascular disease and 11.8 % had malignancy. 51 % of the patients underwent operations. 92.2 % of the patients was hospitalized and the average length of stay was 42.8 days. The average time between admission date and MRSA isolation was 18 days. All outpatients underwent orthopedic operations in the past three months. All isolated strains (100 %) were regarded as the causes of nosocomial infection or healthcare associated infecton. Eighteen strains (35.3 %) were isolated from wounds, 13 (25.5 %) from blood, 11 (21.6 %) from tracheal aspirate, two (3.9 %) from sputum, two (3.9 %) from operation material, two (3.9 %) from urine, one (2 %) from catheter, one (2 %) from peritoneal fluid, one (2 %) from ple- ural fluid. All MRSA strains were susceptible to vancomycin, teicoplanin, linezolid and daptomycin. MIC50/MIC90 values of antibiotics were 0.75/1.5 μg/ml for vancomycin, 2/4 μg/ml for teicoplanin, 0.5/1 μg/ml for linezolid and 0.125/0.5 μg/ml for daptomycin. Treatment was not given in seven patients (13.7 %) because isolated pathogens were accepted as colonisation. Five (9.8 %) patients had died before culture results were reported, therefore appopriate treatment could not be given. Thirty nine patients were treated and treatment response rate was 61.5 %. 20 patients (51.3 %) who received treatment died. Twenty (51.3 %) of the treated patients had died. The factors that increased mortality were patients age of ≥65 years, mechanical ventilation used and ≥72 hours for delayed antibiotherapy. Finally all strains were susceptible to vancomycin, teicoplanin, linezolid and daptomycin but linezolid and daptomycin MIC values were lower.
