Karbapenemlere dirençli Acınetobacter baumannıı suşlarında metallo-beta-laktamaz varlığının araştırılması

Acinetobacter türleri antibiyotiklere karşı çoğul direnç geliştirmeleri nedeniyle önemli hastane infeksiyonu etkenleridir. Acinetobacter kökenlerinde beta-laktam antibiyotiklere karşı olan direnç büyük oranda beta-laktamaz üretimine bağlıdır. Metallo-beta-laktamaz (MBL) aktivitesi gösteren IMP ve VIM enzimleri aztreonam dışındaki tüm beta-laktam antibiyotikleri hidroliz edebilme yeteneğindedir. Bu çalışmanın amacı, karbapenemlere dirençli A.baumannii suşlarında MBL enzimlerinin varlığının araştırılmasıdır. Çeşitli klinik örneklerden izole edilen karbapenemlere dirençli A.baumannii suşları çalışmaya alınmıştır. Bakteri identifikasyonu konvansiyonel yöntemler, Phoenix 100 BD (Becton Dickinson, Sparks) ve VITEK 2 (bioMerieux, Fransa) otomatize sistemleri kullanılarak yapılmıştır. Antibiyotik duyarlılık testi Clinical Laboratory Standards Institute önerileri dikkate alınarak Kirby-Bauer disk difüzyon yöntemi ile çalışılmıştır. MBL varlığını araştırmak için kombine disk yöntemi (KDY) uygulanmıştır. KDY ile MBL ürettiği saptanan suşlarda blaIMP ve blaVIM genlerinin varlığı polimeraz zincir reaksiyonu (PCR) ile araştırılmıştır. Karbapenem dirençli 202 A.baumannii suşunun 139’unda (% 69) KDY ile MBL üretimi saptanmıştır. PCR ile 139 suşun hiçbirinde blaIMP ve blaVIM genleri tespit edilmemiştir. Bu sonuçlar hastanemizdeki A.baumannii suşlarında karbapenem direncinden sorumlu enzimlerin IMP ve VIM olmadığını göstermiştir. MBL üreten bakterilerin aztreonam dışında tüm beta-laktamlara dirençli olması ve tedavide kullanılabilecek bir MBL inhibitörünün bulunmaması epidemiyolojik açıdan MBL varlığının tespit edilmesini önemli kılmaktadır. Bundan sonraki amacımız diğer MBL enzimlerinin varlığını araştırmak olacaktır.

Investigation of metallo-beta-lactamases in Acinetobacter baumannii strains resistant to carbapenems

Acinetobacter species are important agents of nosocomial infections as they develop multiple resistance to antibiotics. Resistance to beta-lactam antibiotics in Acinetobacter strains depends largely on the production of beta-lactamases. IMP and VIM enzymes indicating metallo-beta-lactamase (MBL) activity are able to hydrolyze all beta-lactam antibiotics except aztreonam. The aim of this study was to investigate the presence of MBL enzymes in A.baumannii strains resistant to carbapenems. A.baumannii strains isolated from various clinical specimens resistant to carbapenems were included in this study. The isolates were identified by conventional methods, Phoenix 100 BD (Becton Dickinson, Sparks) and VITEK 2 (bioMerieux, France) automated systems. Antibiotic susceptibility test was performed by Kirby-Bauer disk-diffusion method according to the standards of Clinical and Laboratory Standards Institute. Combined disc method (CDM) was applied to investigate the presence of MBL. The presence of blaIMP and blaVIM genes were investigated by polimerase chain reaction (PCR) in strains detected MBL production by CDM. MBL production was detected by CDM in 139 (69 %) of 202 A.baumannii strains resistant to carbapenems. BlaIMP and blaVIM genes were not identified in any of the 139 strains tested by PCR. These results showed that the IMP and VIM enzymes are not responsible for carbapenem resistance in A.baumannii strains in our hospital. MBL-producing bacteria are resistant to all beta-lactams except aztreonam and absence of MBL inhibitor that may be used for treatment, determination of the presence of MBL is an important epidemiological point of view. Our next goal will be the investigation of other MBL enzymes.
