KARBAPENEM DİRENÇLİ ACINETOBACTER BAUMANNII İZOLATLARINDA ANTİBİYOTİK DİRENCİNİN ARAŞTIRILMASI
Hastanede uzun süre yatan, invazif girişimler uygulanan hastalarda Acinetobacter baumannii’nin neden olduğu ciddi enfeksiyonlar sık görülür. Son yıllara kadar karbapenemler, çoklu ilaca dirençli A.baumannii kaynaklı enfeksiyonları tedavi etmek için seçilen ilaçtı. Ancak karbapenemlere karşı direncin giderek artması yeni tedavi seçenekleri arayışına neden olmuştur. Bu çalışmanın amacı hastanemizde çeşitli örneklerden tanımlanan karbapenem dirençli A.baumannii izolatlarının antibiyotik direnç oranlarının belirlenmesidir. Bu çalışmada Ocak 2016-Temmuz 2017 tarihleri arasında Gaziosmanpaşa Üniversitesi Tıp Fakültesi Araştırma ve Uygulama Hastanesi Mikrobiyoloji Laboratuvarı’na çeşitli kliniklerden gönderilen örneklerden izole edilen enfeksiyon etkeni olarak kabul edilen 237 karbapenem dirençli A.baumannii izolatının antibiyotik direnç oranları retrospektif olarak incelenmiştir. İzolatların tanımlanmasında VITEK 2 (bioMérieux, Fransa) cihazı kullanılmıştır. Antibiyotik duyarlılıkları Clinical and Laboratory Standards Institute ve European Committee on Antimicrobial Susceptibility Testing önerileri doğrultusunda belirlenmiştir. Hastaların 133’ünün (% 56.1) erkek, 104’ünün (% 43.9) kadın ve yaş ortalamasının 66.1 olduğu görülmüştür. İzolatların 99’unun (% 41.8) solunum yolu, 84’ünün (% 35.4) kan, 33’ünün (% 13.9) yara, 16’sının (% 6.7) idrar, beşinin (% 2.1) diğer örneklerden tanımlandığı tespit edilmiştir. İzolatlara karşı en etkili antibiyotikler % 0.4 direnç oranı ile kolistin ve % 23.9 direnç oranı ile tigesiklin olarak saptanmıştır. Yoğun bakımdan ve servislerden izole edilen A. baumannii izolatları arasında antibiyotik direnci açısından fark tespit edilmemiştir. Son yıllarda karbapeneme dirençli A.baumannii izolatlarının yaygınlaşmasının önlenmesi için uluslararası stratejilerin geliştirilmesi gündeme gelmiştir. Bu amaçla Dünya Sağlık Örgütü hastane ve diğer sağlık kuruluşlarında sürveyans çalışmalarının yapılması gerektiğini vurgulamıştır. Investigation of Antibiotic Resistance in Carbapenem Resistant Acinetobacter baumannii Isolates
Severe infections caused by Acinetobacter baumannii are common in patients with long-standing hospitalizations and, invasive procedures in the hospital.Until recent years, carbapenems have been the drugs of choice for the treatment of infections with A.baumannii. However, increasing resistance to carbapenems has led to the search for new treatment options. The aim of this study is to determine the antibiotic resistance rates of carbapenem resistant A.baumannii isolates identified in various samples in our hospital. In this study, antibiotic resistance rates of 237 carbapenem resistant A.baumannii isolates identified from samples sent from various clinics to Gaziosmanpasa University Medical Faculty Research and Practice Hospital Microbiology Laboratory between January 2016 and July 2017 were examined retrospectively. VITEK 2 (bioMérieux, France) instrument was used to identify the isolates. Antibiotic susceptibility was determined by the Clinical and Laboratory Standards Institute and the European Committee on Antimicrobial Susceptibility Testing. It is known that 133 (56.1%) of the patients are male, 104 (43.9 %) are female and age average of them is 66.1. It was determined that 99 (41.8 %) of the isolates were defined as respiratory tract, 84 (35.4 %) blood, 33 (13.9 %) wound, 16 (6.7 %) urine, and five (2.1 %) other samples.The most effective antibiotics against isolates were tigecycline with a resistance rate of 0.4% and colistin with a resistance rate of 23.9%. There was no difference in antibiotic resistance between A.baumannii isolates isolated from intensive care units and services. In recent years, the development of international strategies has been on the rise to prevent the spread of carbapenem resistant A.baumannii isolates. For this purpose, World Health Organization emphasized that surveillance studies should be done in hospitals and other health institutions. Our high resistance rates show the dimensions of the danger and give an idea of the picture we may encounter in the future. This study showed that colistin and tigecycline could be alternatives in the treatment of carbapenem resistant A.baumannii infections in our hospital.
