HBsAg POZİTİF HASTALARIN TROMBOSİT İNDEKSLERİNİN DEĞERLENDİRİLMESİ

Hepatit B virüsü, dünya genelinde kronik karaciğer inflamasyonunun major nedenidir. HBV ile ilişkili karaciğer hastalıklarında trombositlerin klinik önemi birçok çalışmada gösterilmiştir. Trombositlerin hemostazdaki fonksiyonları iyi bilinmesine rağmen, kronik inflamasyon durumlarında inflamatuar tetikleyici olarak önemli rollerini destekleyen çalışmalar yakın zamanda artmıştır. Trombositler aktive olduklarında boyutlarını, çok çeşitli biyoaktif inflamatuar ve prokoagülan moleküllerin salgılanmasını ve çeşitli inflamatuar reseptörlerin ekspresyonunu arttırırlar. Bu çalışmada, HBsAg pozitif hastalarda platelet sayısı ve platelet indeksleri olan ortalama trombosit hacmi (MPV), trombosit dağılım genişliği (PDW), kandaki trombosit yüzdesi (PCT) parametreleri değerlendirilmiştir. Yaş ve cinsiyetleri eşleştirilen 160 HBsAg pozitif ve 160 HBsAg negatif hasta bu çalışmaya dahil edilmiştir. HBsAg testi Mikro ELISA, tam kan sayımı elektrik impedans yöntemi ile çalışılmıştır. Trombosit sayısı ve PCT değerlerinin HBsAg pozitif grupta HBsAg negatif gruba göre istatiksel olarak daha düşük olduğu tespit edilmiştir (sırasıyla p= 0.001 ve p= 0.008). MPV HBsAg pozitif grupta daha yüksek saptanmasına rağmen, bu artış istatiksel olarak anlamlı bulunmamıştır. HBsAg pozitif grupta tespit edilen yüksek PDW, istatiksel olarak anlamlı bulunmuş- tur (p= 0.016). Sonuç olarak, HBsAg pozitif hastaların trombosit aktivasyonunda artış tespit edilmiştir. Bundan yola çıkarak basit, ucuz ve rutinde sıklıkla kullanılan trombosit parametrelerinin karaciğer inflamasyonu ile ilgili önemli bilgiler sağlayabileceği düşünülmüştür.

Evaluation of Platelet Indices of HBsAg Positive Patients

Hepatitis B virus (HBV) is a major cause of chronic liver inflammation world wide. The clinical significance of platelets in HBV-related liver diseases has been shown in many studies. Although platelets are well known for their functions in hemostasis, studies supporting their considerable role as an inflammatory trigger in chronic inflammatory conditions have increased. When platelets activate, they increase in size, release a great variety of bioactive inflammatory and procoagulant molecules, and express a variety of inflammatory receptors. In this study, platelet count and platelet indices including mean platelet volume (MPV), platelet distribution width (PDW) and platelet-crit (PCT) were evaluated in HBsAg-positive patients. A hundred and sixty HBsAg positive and 160 HBsAg negative patients matched for age and sex were included in this study. HBsAg test was performed by micro ELISA, and complete blood count was tested by electrical impedance methods. The platelet count and PCT values in HBsAg-positive group were statistically lower (p=0.001 and p=0.008, respectively) than HBsAg-negative group. Although MPV was higher in HBsAg-positive group, this increase was not statistically significant. The high PDW detected HBsAg-positive group was statistically significant (p=0.016). In conclusion, an increase in platelet activation in HBsAg-positive patients was detected. Platelet parameters, which are simple, inexpensive and commonly used in routine clinical practice, may provide important information about the liver inflammation.
