AYAKTAN BAŞVURAN HASTALARDA ÜRİNER SİSTEM İNFEKSİYONUNA NEDEN OLAN ESHERICHIA COLI VE KLEBSIELLA SPP. SUŞLARININ ÇEŞİTLİ ANTİBİYOTİKLERE DİRENÇ ORANLARININ VE GENİŞLEMİŞ SPEKTRUMLU BETA-LAKTAMAZ VARLIĞININ DEĞERLENDİRİLMESİ

Üriner sistem infeksiyonlarının (ÜSİ) çoğunluğunu komplike olmayan ve toplumdan kazanılmış infeksiyonlar oluşturmaktadır. Escherichia coli ve daha az sıklıkla da Klebsiella spp. suşlarının ilk sıralarda yer alan ÜSİ etkeni oldukları bilinmektedir. Bu bakterilerin geliştirmekte olduğu direnç tüm dünya için önemli bir sorun teşkil etmektedir. Direnç oluşumundaki en önemli mekanizmalardan birisi genişlemiş spektrumlu beta-laktamaz (GSBL) üretimidir. Bu çalışmada idrar kültürlerinden izole edilen E.coli ve Klebsiella spp. suşlarının çeşitli antibiyotiklere direnç oranları ve GSBL varlığının, sık kullanılan antibiyotiklerdeki direnç oranına etkisi retrospektif olarak araştırılmıştır. Çeşitli polikliniklerden laboratuvarımıza gönderilen idrar örneklerinde üreyen bakterilerin tanımlanmasında konvansiyonel yöntemler ve/veya otomatize VITEK 2 Compact System (BioMérieux, Fransa) kullanılmıştır. E.coli ve Klebsiella spp. olarak tanımlanan suşların antibiyotik duyarlılıkları Kirby-Bauer disk difüzyon yöntemi ve/veya otomatize VITEK 2 Compact System ile, GSBL varlığı ise kombine disk difüzyon yöntemi ile araştırılmıştır. Laboratuvarımıza çeşitli polikliniklerden toplam 29,165 idrar örneği gönderilmiş olup 2,803’ünde (% 10) anlamlı bakteri üremesi saptanmıştır. Üreyen bakterilerden 2,239’u (% 80) E.coli, 271’i (% 10) Klebsiella spp. olarak tanımlanmıştır. Ampisilin, amoksisilinklavulanik asit, sefazolin, seftriakson ve trimetoprim-sülfametoksazol için her iki cinsin de % 20’nin üzerinde direnç gösterdiği tespit edilmiş- tir. Piperasilin-tazobaktam direnci E.coli suşlarında % 19, Klebsiella spp. suşlarında % 30 olarak bulunmuştur. E.coli suşlarında % 14 olan nitrofurantoin direncinin Klebsiella spp. suşlarında % 52 olduğu görülmüştür. GSBL varlığı E.coli suşlarının % 20’sinde, Klebsiella spp. suşlarının ise % 27‘sinde saptanmıştır. GSBL pozitif olan suşlarda antibiyotik direnç oranlarının GSBL üretmeyen suşlara göre daha yüksek olduğu tespit edilmiştir. Sonuç olarak çalışmamızda E.coli en sık saptanan etken olup, bu suşların nitrofurantoin, kinolonlar ve aminoglikozitlere % 20’nin altında direnç gösterdiği belirlenmiştir. Bu nedenle bu antibiyotiklerin ampirik tedavide tercih edilebilir olduğunu düşünmekteyiz. Ancak hem antibiyotiklerin olumsuz etkileri hem de yüksek GSBL pozitifliği ve bu suşlarda antibiyotiklere artmış direnç nedeniyle tedavi öncesi kültür ve antibiyogram testi gerekli görünmektedir.

The Evaluation of Resistance Rates to Various Antibiotics and Extended Spectrum Beta-lactamase Positivity of Esherichia coli and Klebsiella spp. Strains Isolated from Outpatients with Urinary Tract Infection

Most of the urinary tract infections are uncomplicated and community acquired. Escherichia coli and less frequently Klebsiella spp. strains are known to be the most frequent microorganisms that cause urinary tract infections. Development of antimicrobial resistance in these bacteria is an important problem for the world. The most important mechanism for antibacterial resistance ise production of extended spectrum beta-lactamases (ESBL). In this study antimicrobial resistance rates of E.coli and Klebsiella spp. strains isolated from urine cultures and impact of the ESBL positivity on the resistance rate of frequently used antibiotics have been evaluated retrospectively. Conventional methods and/or automated VITEK 2 Compact System (BioMérieux, Fransa) were used for identification of bacteria obtained from urine samples coming from various clinics. Antimicrobial susceptibility of bacteria identified as E.coli and Klebsiella spp. was determined by Kirby-Bauer disk diffusion method and/or automated VITEK 2 Compact System. A total of 29,165 urine samples were sent to our laboratory from outpatients. Significant bacterial growth was detected in 2,803 (80 %) of the specimens. Of the isolated bacteria, 2,239 were identified as E.coli (80 %) and 271 were identified as Klebsiella spp. (10 %). Resistance rates of both E.coli and Klebsiella spp. were over 20 % for ampicillin, amoxicillin-clavulanic acid, cefazolin, ceftriaxone and trimethoprimsulfamethoxazole. Resistance to piperacillin-tazobactam was 19 % for E.coli, 30 % for Klebsiella spp. Nitrofurantoin resistance rate of E.coli strains were 14 % and Klebsiella spp. strains were 52 %. The prevalance of ESBL production was 20 % for E.coli and 27 % for Klebsiella spp. Antibiotic resistance rates were higher in ESBL positive strains than in ESBL negative strains. In conclusion, in our study E.coli is the most detected microorganism and for these strains nitrofurantoin, quinolones and aminoglycosides’ resistance rates were below 20 %. Therefore we think that these antibiotics may be used for empirical therapy. But because of antibiotics’ negative effects and the high ESBL positivity in E.coli strains and the increased resistance rates for the antibiotics in these strains, it seems necessary to perform culture and antibiotic susceptibility testing before therapy.
