Yanık hastalarında anestezi deneyimlerimiz
Amaç: Yanık hastalarının debridman ve greftleme cerrahileri yanık yüzey alanıyla orantılı olarak büyük oranda ısı ve sıvı kaybına neden olur. Çalışmamızın amacı, debridman ve/veya greftleme amacıyla operasyona alınan yanık hastalarındaki anestezi deneyimlerimizi değerlendirmektir. Yöntem: Nisan 2003-Aralık 2005 tarihleri arasında yanık sebebiyle ameliyathanede debridman ve/veya greftleme yapılan hastaların dosyaları ve anestezi kayıtları retrospektif olarak incelendi. Bulgular: 48 hasta toplam 97 defa debridman ve/veya greftleme işlemi için operasyona alındı. Olguların yaş ortalaması 26.5±22.6 (1-90), ortalama yanık yüzdeleri %39.9±23.4 (4-95) idi. Olguların cerrahi başlangıç ve bitimindeki ortalama vücut sıcaklıkları, hemoglobin, hematokrit ve BUN değerlerinde anlamlı fark bulunurken diğer laboratuvar değerleri benzerdi. Cerrahi süresindeki ortalama idrar miktarı 360.5±330.6 mL idi. Cerrahinin başlangıcındaki pH değerlerinin cerrahi sonunda düştüğü görüldü (p=0.017). 25 olguda (%25.8) intraoperatif hipotansiyon geliştiği ve bu olguların 13’ünde (%13.4) sıvı resüsitasyonuna yanıtsızlık nedeniyle inotrop destek gerektiği belirlendi. Yoğun bakım takipleri sırasında 12 hasta (%25) sepsis ve septik şok tanılarıyla kaybedildi. Sonuç: Yanık hastalarında vücut sıcaklığı ve sıvı-elektrolit denge bozuklukları sık karşılaşılan bir klinik tablodur. Deri bütünlüğünün kaybı ve cerrahi sırasındaki fazla kanama ile büyük miktarlarda sıvı, kan ve kan ürünleri replasmanının gerekliliği nedeniyle komplikasyonların üstesinden gelebilmek için çok iyi preoperatif değerlendirme ile dikkatli ve ayrıntılı bir intraoperatif takip gerekmektedir.
Anesthesia management of burn patients
Objective: Debridement and grafting surgery of burn patients may cause excess amounts of fluid and heat loss depending on the surface area of the burned skin. The aim of our study was to evaluate our anesthesia experiences in debridement and grafting surgery of burn patients. Method: The anesthesia recordings of the burn patients, who were scheduled for debridement and grafting surgery between April 2003 and December 2005, were evaluated retrospectively. Results: A total of 97 operations on 48 patients were evaluated. The mean age of the patients was 26.5±22.6 (1-90). The percentage of the burned area was 39.9±23.4% (4-95). Postoperative mean body temperature, hemoglobin, hematocrit, and blood urea nitrogen (BUN) levels were significantly different when compared with the preoperative values. Other biochemical parameters did not change significantly at postoperative measurements. The mean urine amount was 360.5±330.6 mL. The mean basal pH value of the patients was 7.36±0.92 and it decreased to 7.29±0.11 (p=0.017) postoperatively. 25 cases (25.8%) developed intra- operative hypotension and thirteen (13.4%) of these cases needed inotropic support. Even though no perioperative mortality was seen, 12 patients (25%) died due to sepsis and septic shock during the intensive care unit follow up. Conclusion: Disturbances in fluid and electrolyte balance in burn patients are commonly seen mainly due to defects in skin compliance and excessive blood loss in burn surgery. Preoperative evaluation of the patients and careful intraoperative follow up are essential to cope with the complications of burn surgery.
