Pediatrik olgularda laringeal maske yerleştirilmesinde sevofluran ve halotan indüksiyonlarının karşılaştırılması

Çalışmamızda pediyatrik olgularda, laringeal maske (LM) yerleştirilmesinde; sevofluran ve halotan ile anestezi indüksiyonlarının etkilerini karşılaştırmayı amaçladık. Yaşları 6 ay - 6 yaş arasında, ASA I-II grubuna dahil 30 olgu çalışmaya alındı. Operasyondan 30 dakika önce, premedikasyon olarak tüm olgulara 0.5 mg kg-1 rektal midazolam uygulandı. Anestezi indüksiyonu; % 50 O2/N2O ve aşamalı şekilde artırılan sevofluran (S) veya halotan (H) gaz karışımı ile sağlandı. Her iki ajanın konsantrasyonu, kirpik refleksi kayboluncaya kadar her solumada % 0.5 artırıldı (S için maksimum % 8, H için maksimum % 5 konsantrasyon olacak şekilde). Olgulara uygun numara LM yerleştirildi. Vital parametreler ve periferik oksijen satürasyonu noninvaziv olarak belirlendi. Kirpik refleksi kaybolma ve LM yerleştirme süreleri ile tüm komplikasyonlar kaydedildi. Kirpik refleksinin kaybolma ve LM yerleştirme süreleri Grup S'de daha kısaydı. Vital bulgular Grup S'de daha stabil seyretti. İndüksiyonu takiben LM yerleştirilmesi öncesi sistolik ve diyastolik kan basınçları Grup H'de, Grup S'ye göre anlamlı olarak düşük bulundu. Komplikasyonlar bakımından gruplar arasında fark yoktu. İndüksiyon süresinin kısa, havayolu refleks yanıtlarının daha az olması ve kardiyovasküler stabilitenin korunması nedeni ile pediyatrik olgularda LM yerleştirilmesinde sevofluranın halotana alternatif olabileceği kanısına varıldı.

Comparison of sevoflurane and halothane inductions on laryngeal mask airway insertion in pediatric patients

The aim of our study was to compare the effects of sevoflurane and halothane inductions on laryngeal mask airway (LMA) insertion in pediatric patients. Thirty patients in ASA I-II, aged between 6 months-6 years were included in the present study. Thirty minutes before the operation, all patients received 0.5 mg kg<sup>-1</sup> midazolam rectally as premeditation. Anesthesia was induced with 50 % O<sub>2</sub>/N<sub>2</sub>O and incremental doses of sevoflurane (S) or halothane (H). The concentration of the agents were increased 0.5 % with every breath until the loss of eyelash reflex (Maximum concentrations were 8 % for S and 5 % for H). Appropriate LMA was inserted to the patients. Vital parameters, peripheral oxygen saturation, time to loss of eyelash reflex and LMA insertion and all complications were recorded. In Group S, time to loss of eyelash reflex and LMA insertion were shorter than Group H. Vital parameters were more stable in Group S when compared with the other group. After the induction, systolic and diastolic blood pressures before LMA insertion were significantly lower in Group H than Group S. Complications did not differ between the groups. We concluded that; sevoflurane can be an alternative to halothane in pediatric patients for LMA insertion because of short induction period, providing less answer to airway reflexes and protecting cardiovascular stability.
