Ortalama Trombosit Hacmi Kardiyak Arest Sonrası Hedefe Yönelik Hipotermi Uygulanan Hastalarda Mortalite için Belirteç Olarak Kullanılabilir mi?
Amaç: Mevcut kılavuzlar, kardiyak arest sonrası (PKA) hastalarda klinik sonuçları iyileştirmek adına orta derecede hedefe yönelik sıcaklık tedavisi (HYH) önermektedir. HYH alan hastalarda sağkalım ile ilgili bir prognostik değer tam olarak belirnememiştir. Bu çalışma, HYH uygulanan PKA hastalarında ortalama trombosit hacmi (OTH) ile mortalite arasındaki ilişkiyi değerlendirme- yi amaçlamıştır. Yöntem: Retrospektif olarak tek merkezde yapılan bu çalışmamıza, kardiyak arest geçiren ve 6 saat içinde için HYH uygulanan 15 hasta dahil edildi. HYH, 48 saat boyunca 34°C’lik bir hedef sıcaklıkta tutuldu. İlk 96 saat içinde PKA hastalarının karakteristik özellikleri ve OTH ile ilgili tüm veriler incelenip kayıt altına alındı. Bulgular: On beş hastanın 11’i (%73.3) hastanede kaldığı süre içinde yaşamını yitirdi. T0, T1, T2, T3, T4 24 saat aralıkları ile toplamda 5 defa hemogram değerlerine bakıldı. Mean Platelet Volume Could it Be Used as a Predictive Marker for Mortality in Post Cardiac Arrest Patients Undergoing Targeted Temperature Management?
Objective: Current guidelines recommend a period of moderate targeted temperature manage- ment (TTM) for post cardiac arrest patients (PCA) to improve clinical outcomes. A prognostic value related to survival in patients receiving TTM was not fully established. In this study, we aimed to investigate the relationship between mean platelet volume (MPV) and mortality in PCA patients undergoing TTM. Method: This was a single-center, retrospective cohort study included 15 patients who suffered from cardiac arrest and underwent TTM within 6 hours of cardiac arrest. Hypothermia was main- tained for 48h at a target temperature of 34°C. We examined all data about characteristic fea- tures of PCA patients s as well as MPV within the first 96 hours from admission. Results: Of the 15 patients, 11 (73.3%) died during hospital stay. At time points of T0, T1, T2, T3, T4 24 hour ie. a total of 5 times hemogram values were determined.
