Multiple sklerozis (MS) atağı sırasında genel anestezi
Giriş: Beyin ile spinal kordun yavaş, ilerleyici, akkiz, demiyelinizan bir hastalığı olan Multipl Skleroz (MS); ataklar ve remis- yonlarla seyreder. Duygu durum değişiklikleri, cerrahi/anestezik travma, özellikle de enfeksiyona bağlı ateş sıcağa duyarlılıkları artmış demiyelinize aksonları etkileyebilir ve mevcut hastalığı agreve edebilir. Olgu: 8 yıldır MS tanısı ile izlenen, 45 yaşında erkek hasta akut apandisit ön tanısı aldı ve acil operasyon için preoperatif değerlendirildi. Hastanın son 4 gündür iştahsızlık, karın ağrısı ve ateş şikayetleri vardı. ‹ki gündür de görmede bulanıklık, konuşma bozukluğu, denge kaybı mevcuttu. Koltuk altından ölçülen vücut ısısı 38,8 °C ve beyaz küre sayısı 16.000 idi. Başka bir preoperatif özellik ile karşılaşılmadı. Düzenli ilaç tedavisi almayan hastada enfeksiyonun tetiklediği bir akut MS atağı düşünüldü ve hasta operasyon odasına alındı. Hasta elektrokardiyografi (EKG), noninvaziv kan basıncı ve O2 saturasyonu ile monitorize edildi. ‹ndüksiyon için 1 mg kg-1 metil prednizolon, 1µg kg-1 fentanil, 6 mg kg-1 tiyopental ve 0,6 mg kg-1 rokuronyum kullanıldı. Hasta entübe edildi. Anestezi idamesi, % 50 N2O-O2 ve % 5 desfluran ile sağlandı. Operasyon boyunca 5 dakika aralıklarla timpanik membrandan vücut ısısı ölçüldü ve periferik soğutma teknikleri uygulandı. Kas gevşetici etki antagonize edilmeden sorunsuz uyandırılan hastada, postoperatif dönemde herhangi bir komplikasyona rastlanmadı. Hasta cerrahi ve nöroloji kliniklerince 15 gün takip edildi. Enfeksiyonu geriledikten sonra, tedavisi düzenlenerek remisyonla taburcu edildi. Sonuç: Literatür incelendiğinde, atak sırasında desfluran ile idamesi yapılan MS’li hasta örneğine rastlanmadı. Bu hastaların anestezilerinde, mevcut tedavilerine devam edilmelidir. Antikolinerjik ajanlardan kaçınılmalı, nondepolarizan kas gevşeticiler ter- cih edilmelidir. Vücut sıcaklığı değişiklerine izin verilmemeli, özellikle de hipertermi önlenmelidir.
General anesthesia for a patient with multiple sclerosis (MS) at exacerbation stage
Introduction: Multiple sclerosis is an acquired, slowly progressive, demyelinating disease of the brain and the spinal cord. It is usually characterized with remissions and exacerbations. Changes in mood, surgical/anesthesic trauma and especially fever due to infection can aggravate the disease by affecting the demyelinated axons with increased sensitivity to heat. Case: A 45 year old man who had been diagnosed with MS was evaluated for emergency surgery due to suspected acute appendicitis. The patient had experienced lack of appetite, abdominal pain and fever complaints for the last 4 days. He had had blurred vision, speech disorder and loss of equibilirium for 2 day prior to admission. The patient had no special complaints preoperatively except 38,8 °C axillary fever and white blood cell count of 16,000. The patient was on no regular medication and considering an MS attack can be triggered by infection. He was taken into the operating room. After EKG, non-invasive blood pressure and oxygen saturation monitorization, the induction was done with 1 mg kg-1 methylprednisolone, 1 µg kg-1 fentanyl, 6 mg kg-1 tiopenthal and 0,6 mg kg-1 rocuronium. The patient was intubated and the anesthesia maintenance was done with % 50-% 50 N20 and O2 and % 5 desflurane. The body heat was measured at 5 minute intervals and peripheric cooling techniques were used during the procedure. The patient was extubated without reversal of the neuromusculer block when spontaneus ventilation was adequate and no complication has accured afterwards. The patient was observed at surgical and neurological units and discharged after 15 days. Conclusion: In our review of the literature, no desflurane use for anesthesia management of MS in the acute phase was found. For the anesthesia of such patients the current treatment should be continued. No reverse agents should be used and non depolarizing agents should be choosen. The body temperature should not be allowed to change and hyperthermia should be prevented.
