Moebius sendromlu hastada anestezik yaklaşım
Moebius Sendromu, fasial (VII) ve abdusens (VI) sinirlerinin unilateral veya bilateral konjenital paralizisiyle karakterize nadir bir hastalıktır. Bu sendromun klasik özellikleri, orofasiyal malformasyonlar, kas-iskelet defektleri, kardiyak malformasyonlar ve mental-motor retardasyondur. Hastalar fasiyal paraliziye bağlı maske yüz, salya akması, göz kapağının tam kapanamaması ve abdusens paralizisine bağlı strabismusla tanınırlar. ‹skelet kaslarının hipotonisine bağlı yutma fonksiyonunun bozulması, oral sekresyonların aşırı artması ve orofasiyal anormallikler rutin havayolu yaklaşımını zorlaştırabilmekte, entübasyon süresinin uzaması intraoperatif aspirasyon riskini artırabilmektedir. Bu olguda, Moebius Sendromlu 19 aylık erkek çocuğa göz operasyonu esnasında uygulanan anestezik yaklaşım anlatılmıştır.
Anaesthetic management of Moebius syndrome
Moebius syndrome is a rare disorder, that is characterized by unilateral or bilateral congenital paralysis of the facial (VII) and abducens (VI) nerves. Orofacial malformations, musculoskeletal defects, cardiac malformations and mental-motor retardation are classic features of this syndrome. Patients are characterized as having mask-like faces, drooling, incomplete eyelid closure due to facial paralysis and strabismus due to abducens paralysis. Defective swallowing due to skeletal muscle hypotonia may lead to excessive oral secretions. Orofacial abnormalities may cause routine airway management and intubation time may increase with intraoperative aspiration risk. Anesthetic management of a 19-month-old male infant with Moebius syndrome is reported.
